Abstract
The present study examined the prevalence and factors associated with depressive episode in primary caregivers of schizophrenic patients of the Psychiatric Outpatient Department, Siriraj Hospital, Bangkok. A total of 88 primary caregivers were enrolled. Data were collected by questionnaires and interviews including the demographic characteristics questionnaires of caregivers, The Zarit Burden Interview (ZBI), The Personal Resource Questionnaire: PRG 85 - Part 2, The Thai quality of life in health questionnaires (Medical Outcomes Study Short form: SF-36), Clinical interview for psychiatric diagnosis (Mini International Neuropsychiatric Interview: MINI) and Hamilton Rating Scale for Depression: HRSD. The data was analyzed by frequency, percentage, mean and standard deviation. For relativity analysis, were used Pearson's correlation coefficient, Chi-square test and Fisher's Exact test. For comparison, the Mann-Whitney U or t-test independent was used and the risk factors analysis of caregivers depression were done by stepwise forward logistic regression and adjusted odds ratio. Results showed that the prevalence of the caregiver's depression was 11.4% (n = 10); Major Depressive Episode was 4.5% (n = 4) and Dysthymia was 6.8% (n = 6). There were 2 statically significant factors which were associated with the caregivers' depression; the quality of life in mental health of caregivers (Adjusted OR .919) and the medical problem of caregivers (Adjusted OR 18.65). In addition, the relationship of 3 factors including burden of caregivers, social support and quality of life in health were all significantly associated. Social support and quality of life in health were negatively low related with the burden of caregivers. The social support was positively low related with the quality of life in health. From the results of the study, it is important to be aware of the prevalence and factors associated with depressive episodes among the caregivers of the patients with schizophrenia, these are; the quality of life in mental health of caregivers and the medical problem of caregivers. This would be beneficial information to help the caregivers.
การศึกษาครั้งนี้เป็นการศึกษาความชุกและปัจจัยที่เกี่ยวข้องกับภาวะซึมเศร้าในผู้ดูแลผู้ป่วยจิตเภท กลุ่มตัวอย่างคือ ผู้ดูแลผู้ป่วยจิตเภทที่พาผู้ป่วยมารับการตรวจที่หน่วยจิตเวชศาสตร์ อาคารผู้ป่วยนอก โรงพยาบาลศิริราช กรุงเทพมหานคร จำนวนทั้งหมด 88 คน เก็บข้อมูลโดยแบบสอบถามและแบบสัมภาษณ์ ซึ่งประกอบด้วย ข้อมูลทั่วไปของผู้ดูแล, แบบประเมินความรู้สึกเป็นภาระของผู้ดูแล (ZBI), แบบประเมินแรงสนับสนุนทางสังคม, แบบประเมินคุณภาพชีวิตด้านสุขภาพ (SF-36), แบบสัมภาษณ์ M.I.N.I. และแบบประเมินความรุนแรงของภาวะซึมเศร้า (HRSD) โดยวิเคราะห์ข้อมูลด้วยความถี่ ร้อยละ ค่าเฉลี่ย และส่วนเบี่ยงเบนมาตรฐาน วิเคราะห์ความสัมพันธ์ด้วยค่าสัมประสิทธิ์สหสัมพันธ์ของเพียร์สัน Chi-square test และ Fisher's Exact test เปรียบเทียบความแตกต่างด้วยสถิติ Mann-Whitney U หรือ t-test independent และวิเคราะห์ปัจจัยเสี่ยงภาวะซึมเศร้าของผู้ดูแลด้วยการวิเคราะห์การถดถอยพหุแบบลอจิสติก (stepwise forward logistic regression) ผลการวิจัยพบว่า ความชุกของภาวะซึมเศร้าในผู้ดูแลผู้ป่วยโรคจิตเภทในการศึกษาครั้งนี้คือ 10 คน (11.4%) โดยเป็น Major Depressive Episode จำนวน 4 คน (4.5%) และ Dysthymia จำนวน 6 คน (6.8%) และพบว่ามี 2 ปัจจัยที่เกี่ยวข้องกับภาวะซึมเศร้าในผู้ดูแลผู้ป่วยจิตเภทคือ คุณภาพชีวิตด้านสุขภาพจิต (Adjusted OR .919) และการมีโรคประจาตัวของผู้ดูแล (Adjusted OR 18.65) นอกจากนี้ ปัจจัยความรู้สึกเป็นภาระของผู้ดูแล แรงสนับสนุนทางสังคมและคุณภาพชีวิตด้านสุขภาพของผู้ดูแล พบว่ามีความสัมพันธ์กันทั้ง 3 ปัจจัย โดยแรงสนับสนุนทางสังคมและและคุณภาพชีวิตด้านสุขภาพมีความสัมพันธ์ทางลบในระดับต่ำกับความรู้สึกเป็นภาระของผู้ดูแล และแรงสนับสนุนทางสังคมมีความสัมพันธ์ทางบวกในระดับต่ำกับคุณภาพชีวิตด้านสุขภาพ ผลการศึกษาทำให้ทราบถึงความชุกของภาวะซึมเศร้าในผู้ดูแลผู้ป่วยจิตเภทและทราบถึงปัจจัยที่เกี่ยวข้องกับภาวะซึมเศร้าในผู้ดูแลผู้ป่วยจิตเภท คือคุณภาพชีวิตด้านสุขภาพจิตและการมีโรคประจาตัวของผู้ดูแล ซึ่งจะเป็นข้อมูลที่เป็นประโยชน์ต่อการดูแลช่วยเหลือผู้ดูแลผู้ป่วยต่อไป
