Abstract
This qualitative study explored the role of decision making by health service providers at the STD clinic of Kalubowila, Sri Lanka and by the adolescents who have visited this clinic in terms of facilitating and asserting the right of adolescents to access HIV testing and know their status. The primary data for this study were gathered through a documentary research of 10 national policies, circulars, and strategic plans with a direct linkage to sexual and reproductive health of adolescents. Primary data were also gathered through semi-structured interviews with 8 health service providers including 4 doctors, 2 nursing staff members, and 2 Saukya Karaya Sahayaka Staff members of the STD clinic of Kalubowila. nine adolescents, who have visited the clinic, were also interviewed including 3 girls, 3 transgender girls, and 3 boys. It is the position of this qualitative study that the current decision-making process by health service providers and adolescents do not significantly contribute to assert the adolescents' right to access available HIV testing at the STD clinic of Kalubowila and know their status. The study also concluded that the current policy framework in the country related to sexual and reproductive health of adolescents do comprehensively facilitate the right to health of adolescents, but do not provide health service providers with tools and avenues to facilitate adolescents right to access HIV testing and know their status. This qualitative study recommends an evaluation of the existing policy framework in the country related to sexual and reproductive health of adolescents in line with international human rights instruments especially including the article 12 of ICESCR, article 24 of CRC, and the General Comment No 14 of CESCR. The study also recommends that National STD AIDS Control Program, to develop standard operating procedures for the clinic to provide HIV and other SRH services to adolescents and to create platforms for health service providers and adolescents to engage outside the patient-doctor dynamic.
