การใช้บริการแพทย์แผนไทยของผู้ป่วยเบาหวานชนิดที่ 2 ในโรงพยาบาลชุมชน จังหวัดปทุมธานี

Abstract

การใช้บริการแพทย์แผนไทยในโรงพยาบาลชุมชนของผู้ป่วยเบาหวานยังมีความเข้าใจไม่ชัดเจน การวิจัยแบบภาคตัดขวางนี้มีวัตถุประสงค์เพื่ออธิบายการใช้บริการแพทย์แผนไทยของผู้ป่วยเบาหวานชนิดที่ 2 ในโรงพยาบาลชุมชน และวิเคราะห์ปัจจัยที่สัมพันธ์กับการใช้บริการแพทย์แผนไทยในโรงพยาบาลชุมชน จังหวัดปทุมธานี กลุ่มตัวอย่างคือ ผู้ป่วยเบาหวานชนิดที่ 2 ในแผนกผู้ป่วยนอกของโรงพยาบาลชุมชน 2 แห่ง จำนวน 260 คน สุ่มตัวอย่างแบบกลุ่ม เก็บข้อมูลเดือนธันวาคม 2563 ถึง กุมภาพันธ์ 2564 โดยแบบสอบถามแบบให้ตอบด้วยตนเอง ความเชื่อมั่นแบบสอบถามความพึงพอใจเท่ากับ 0.83 แบบวัดการเข้าถึงบริการเท่ากับ 0.87 วิเคราะห์ข้อมูลโดยสถิติเชิงพรรณนา สถิติไคสแควร์ สถิติถดถอยโลจิสติกอย่างง่าย และสถิติถดถอยพหุไบนารีโลจิสติก ที่ระดับนัยสำคัญ 0.05 ผลวิจัยพบว่า อัตราการใช้บริการแพทย์แผนไทยในแผนกการแพทย์แผนไทย โรงพยาบาลชุมชน เท่ากับร้อยละ 27.3 ปัจจัยที่สัมพันธ์กับการใช้บริการแพทย์แผนไทยในโรงพยาบาลชุมชน จากผลวิเคราะห์การถดถอยพหุไบนารีโลจิสติก ได้แก่ การเข้าถึงบริการแพทย์แผนไทย (AOR = 12.65, 95% CI 6.24-25.66) การได้รับข้อมูลด้านการแพทย์แผนไทย (AOR = 2.51, 95% CI 1.04-6.10) และไม่ได้ประกอบอาชีพ (AOR = 2.32, 95% CI 1.12-4.79) ข้อเสนอแนะจากผลวิจัยนี้ ผู้กำหนดนโยบายระดับจังหวัดสามารถเพิ่มการใช้บริการแพทย์แผนไทยในโรงพยาบาลชุมชนได้โดยหามาตรการสนับสนุนการเข้าถึงบริการในด้านความสะดวก และด้านความพร้อมในการจัดบริการ

The use of Thai traditional medicine services in community hospitals among type 2 diabetes mellitus patients has remained unclear. This cross-sectional study aims to explain the rate of using Thai traditional medicine services (TTMS) among type 2 diabetes (T2DM) patients in community hospitals in Pathum Thani province and to determine the factors associated with the use of TTMS in community hospitals among these patients. A total of 260 T2DM patients at the outpatient department (OPD) were selected by cluster sampling, in which patients were randomly selected from each community hospital. Data were collected using a self-administered questionnaire from December 2020 to February 2021. The questionnaire revealed that the Cronbach's alpha for satisfaction with the OPD was 0.83 and for access to TTMS was 0.87. Statistical analysis was performed using descriptive statistics, the chi-square test, simple logistic regression, and multiple logistic regression analysis at a 0.05 level of significance. The results showed that the rate of TTMS use among T2DM patients in community hospitals in the last year was 27.3%. In the adjusted multiple logistic regression, access to the services (AOR = 12.65, 95% CI 6.24–25.66), receiving information/perceived TTMS (AOR = 2.51, 95% CI 1.04–6.10), and being unemployed (AOR = 2.32, 95% CI 1.12–4.79) were significantly associated with TTMS use. In conclusion, a high rate of TTMS use was found. For the recommendation, it is suggested that policy makers at the provincial level increase the use of TTMS and focus on improving access to TTMS at the community hospitals in Pathum Thani province.
