Abstract
Scenario: A 14-year-old female presents to the hospital emergency department with acute-onset lower abdominal pain. She is pale and clearly in pain. The patient reports the pain started acutely 48 hours ago and was so severe at the onset that she vomited several times. On examination tenderness is noted in the lower abdomen. Vital signs show no fever evident, and blood tests are normal apart from a raised white cell count.
