ผลของโปรแกรมการฟื้นฟูสภาพโดยเร็วต่อความสามารถในการปฏิบัติกิจวิตรประจำวันของผู้ป่วยหลังผ่าตัดเปิดช่องท้อง

Abstract

การวิจัยครั้งนี้เป็นการวิจัยกึ่งทดลอง เพื่อศึกษาผลของโปรแกรมการฟื้นฟูสภาพโดยเร็วต่อความสามารถในการปฏิบัติกิจวัตรประจำวันของผู้ป่วยหลังผ่าตัดระบบทางเดินอาหารแบบเปิดช่องท้อง โดยใช้กรอบแนวคิดความสำเร็จตามจุดมุ่งหมายของคิงในผู้ป่วยหลังผ่าตัดระบบทางเดินอาหารแบบเปิดช่องท้องที่นัดหมายล่วงหน้า จำนวน 64 ราย เข้ารับการรักษาในหอผู้ป่วยศัลยกรรมทั่วไปชายและหญิง หอผู้ป่วยพิเศษโรงพยาบาลศิริราช เลือกกลุ่มตัวอย่างแบบเฉพาะเจาะจง โดยมีเกณฑ์สำหรับการจับคู่ เก็บข้อมูลในกลุ่มควบคุมซึ่งได้รับการพยาบาลตามปกติก่อน จนครบจำนวน 32 ราย หลังจากนั้นจึงเป็นกลุ่มทดลองโดยการให้โปรแกรมการฟื้นฟูสภาพโดยเร็วร่วมกับการพยาบาลตามปกติ จำนวน 32 ราย รวบรวมข้อมูลโดยใช้แบบประเมินความสามารถในการปฏิบัติกิจวัตรประจำวัน วิเคราะห์ข้อมูลโดยใช้สถิติพรรณนาและสถิติการวิเคราะห์ความแปรปรวนร่วม ผลการศึกษาพบว่าผู้ป่วยหลังผ่าตัดระบบทางเดินอาหารแบบเปิดช่องท้องกลุ่มทดลองมีคะแนนเฉลี่ยความสามารถในการปฏิบัติกิจวัตรประจำวันหลังผ่าตัด ในวันที่ 3 สูงกว่าผู้ป่วยหลังผ่าตัดระบบทางเดินอาหารแบบเปิดช่องท้องกลุ่มควบคุม อย่างมีนัยสำคัญทางสถิติ (p<.05) ข้อเสนอแนะ ควรนำโปรแกรมการฟื้นฟูสภาพโดยเร็วต่อความสามารถในการปฏิบัติกิจวัตรประจำวันของผู้ป่วยหลังผ่าตัดระบบทางเดินอาหารแบบเปิดช่องท้อง ไปประยุกต์ใช้โดยบูรณาการกับการสอนก่อนการผ่าตัด และการดูแลผู้ป่วยหลังผ่าตัดโดยเฉพาะในระยะ 48 ชั่วโมงแรกหลังผ่าตัด เพื่อให้ผู้ป่วยมีการฟื้นฟูสภาพด้านการปฏิบัติกิจวัตรประจำวันได้รวดเร็ว

This study was based on a quasi-experimental research design aimed at studying the effects of early rehabilitation program on activity of daily living (ADL) competency in patients undergoing open abdominal surgery by using King's conceptual framework of success based on set objectives. This study was conducted among 64 patients undergoing scheduled open abdominal surgery who were treated at the General Male and Female Surgical Patients' Ward and the Special Patients' Ward, Siriraj Hospital, with criteria for matching pairs. Data were collected in the control group (32 subjects), who received routine care only, before data were collected from the experimental group (32 subjects), who received the early rehabilitation program and routine care. Data were collected by using the activity of daily living competency evaluation form and analyzed using descriptive statistics and analysis of covariance. According to the findings, the patients undergoing open abdominal surgery from the experimental group had higher mean activity of daily living competency on the third postoperative day than the patients undergoing open abdominal surgery from the control group with statistical significance (p < .05). Recommendations: The early rehabilitation program for activity of daily living competency in patients undergoing open abdominal surgery should be applied and integrated with preoperative instructions and care for postoperative patients, particularly during the first 48 hours after surgery, in order to help patients have early rehabilitation in activity of daily living competency
