Abstract
Mycobacterium species are a group of medically important bacteria, which are major causes of morbidity and mortality around the world, especially since the spread of the human immunodeficiency virus (HIV) world wide. In Thailand, tuberculosis is still the leading cause of death among infectious diseases. Some species of Mycobacterium are always resistant to most antituberculous drugs and the effective treatment depends on each species. Therefore identification of mycobacteria to the species level is important. In general, the differentiation of Mycobacterium species is done by time-consuming biochemical tests. Recently molecular techniques such as the polymerase chain reaction (PCR) has been developed in several microbiological laboratories for the rapid differentiation of mycobacterial species. This study aimed to develop the PCR technique to differentiate Mycobacterium species by amplifying the spacer sequences between 16S rDNA and 23S rDNA gene which has been shown to be species-specific in many microorganisms. 102 strains of mycobacteria belonging to 19 species of the genus Mycobacterium were studied. The rapid-growing mycobacteria could be differetiated from the slow-growing mycobacteria by the presence of the amplified products of 420 to 520 bp and 325 to 370 bp in length respectively. Further digestion of the amplified products with restriction enzymes (HaeIII and BstXI or MspI) enabled differentiation of almost all species studied except M. tuberculosis complex. In addition, four probes have been developed. Two probes, MT164 and MV158, were derived from digestion of the amplified products of M. tuberculosis and M. avium respectively with restriction enzymes to get rid of the 16S rDNA and 23S rDNA sequences. The other two probes, MV21 and MI23, were selected from the highly variable regions within spacer sequences of M. avium and M. intracellulare respectively. The MT164 and MV158 probes showed cross-hybridization with some other mycobacterial species. Whereas MV21 and MI23 probes were highly specific for M. avium and M. intracellulare, respectively. The differentiation of the Mycobacterium species by the polymerase chain reaction (PCR) amplification of the 16S-23S rDNA spacer sequence and restriction enzyme analysis is a simple and rapid method. In the future, this method can be developed in a clinical setting for identifying Mycobacterium species directly form patient specimens by searching for a new pair of primers which is specific to genus Mycobacterium. Moreover the 16S-23S rDNA spacer may be an useful target for the development of Mycobacterium species-specific probe.
เชื้อใน genus Mycobacterium เป็นแบคทีเรีย ที่มีความสำคัญทางการแพทย์มาก เนื่องจากเป็นสาเหตุของ โรคติดเชื้อที่สำคัญที่ทำให้ผู้ป่วยถึงตายได้และยังพบได้ ทั่วโลกรวมทั้งประเทศไทยด้วย โดยเฉพาะอย่างยิ่งเมื่อมี การระบาดของเชื้อ human immunodeficiency virus (HIV) เกิดขึ้น พบว่ามีการเพิ่มจำนวนผู้ป่วยที่ติดเชื้อ นี้มากขึ้น นอกจากนี้เชื้อ mycobacteria บาง species มีการดื้อต่อยาที่ใช้รักษาเกือบทั้งหมดและการรักษายังขึ้น อยู่กับเชื้อแต่ละ species อีกด้วย ดังนั้นห้องปฏิบัติการ ทางวัณโรคจึงต้องวิเคราะห์ให้ทราบ species ที่แท้จริง ของเชื้อ โดยทั่วไปการวิเคราะห์ species ของเชื้อ mycobacteria นิยมใช้วิธีทางชีวเคมีซึ่งยุ่งยากและใช้ เวลานาน วิธีการทางอณูพันธุศาสตร์ เช่น วิธี polymerase chain reaction (PCR) จึงได้มีการพัฒนาเพื่อนำมาใช้ มากขึ้น การศึกษานี้มุ่งที่จะพัฒนาวิธีการแยก species ของเชื้อ mycobacteria โดยวิธี PCR เพิ่มจำนวนส่วน spacer ที่อยู่ระหว่าง 16S rDNA และ 23S rDNA gene ซึ่งมีผู้รายงานว่าสามารถใช้แยก species ของเชื้อ แบคทีเรียชนิดอื่น ๆ ได้ การศึกษานี้ได้ทำการเพิ่มจำนวน ส่วนของ 16S rDNA-23S rDNA spacer ของเชื้อ mycobacteria 19 species จำนวน 102 strains พบว่าสามารถแยกเชื้อ rapid-growing mycobacteria ออกจาก slow-growing mycobacteria ได้ โดยพบว่า ส่วนของ spacer sequence ที่เพิ่มจำนวนขึ้นของ rapid- growing mycobacteria มีขนาดประมาณ 420 ถึง 520 bp ส่วนของ slow-growing mycobacteria มีขนาด ประมาณ 325 ถึง 370 bp หลังจากทำการตัดด้วย restriction enzymes (HaeIII และ BstXI หรือ MspI) พบว่า สามารถ แยกเชื้อ mycobacteria ได้ทุก species ยกเว้นเชื้อใน กลุ่ม M. tuberculosis complex นอกจากนี้ได้ทำการออกแบบ DNA ตรวจสอบ (probe) จำนวน 4 probes เป็น probes ที่ได้จากการตัดชิ้นส่วน spacer ที่ทำการเพิ่มจำนวนขึ้นด้วย restriction enzyme ให้ได้เฉพาะส่วนที่เป็น spacer เท่านั้นจำนวน 2 probes คือ MT164 และ MV158 ซึ่งได้ มาจาก spacer sequences ของเชื้อ M. tuberculosis และ M. avium ตามลำดับ ส่วนอีก 2 probes คือ MV21 และ MI23 ได้จากการคัดเลือกส่วนของ spacer sequences ของเชื้อ M. avium และ M. intracellulare ที่มีความ แตกต่างจาก spacer sequences ของเชื้อ Mycobacterium species อื่น ๆ พบว่า MT164 และ MV158 สามารถเกิด cross-hybridization กับเชื้อ Mycobacterium species อื่นได้ ส่วน MV21 และ MI23 พบว่ามีความจำเพาะต่อเชื้อ M. avium และ M. intracellulare ตามลำดับ วิธีการแยก species ของเชื้อ Mycobacterium โดยการเพิ่มจำนวน 16S-23S rDNA spacer ด้วยวิธี polymerase chain reaction (PCR) แล้วตัดด้วย restriction enzyme ทำได้ง่ายและรวดเร็ว ทั้งยังสามารถ พัฒนาไปใช้ในการตรวจแยกเชื้อโดยตรงจากสิ่งส่งตรวจของ ผู้ป่วยได้ โดยการหา primers คู่ใหม่ที่มีความจำเพาะต่อ เชื้อใน genus Mycobacterium นอกจากนี้ 16S-23S rDNA spacer น่าจะเหมาะที่จะนำมาใช้พัฒนาเป็น probes ที่มีความจำเพาะต่อเชื้อ Mycobacterium species ต่าง ๆ ได้อีกด้วย
