Determinants of childhood overweight in Thailand and their effects on high blood pressure

Abstract

Thailand is now facing an epidemic of childhood overweight and its associated burdens. The objective of this study was to examine the association between childhood weight status and health status. The study employed data from the 4th Thai National Health Examination Survey (NHES IV) 2008-2009. The study found that more than one-half of the sample were living in municipal areas and there were equal proportions of girls and boys in the sample and the mean age was 9 years. The ordered regression analysis found that the factors of having a higher- educated father, living in a municipal area and having a father who was an agriculturist posed the greatest risk of overweight/obese among school-age children. Living in a municipal area, spending less than 1 hour on the computer for learning and watching TV at least 2 hours per day posed the greatest risk of overweight/obese for a single school-age child. In addition, the logistic regression analysis found that obese and overweight children age from 10 to 12 years, had the highest risk of high blood pressure among school-age children and for a single school-age child. Policymakers should consider the various child, parental and environmental characteristics in weight control/reduction programs for the youth.

งานวิจัยครั้งนี้มีวัตถุประสงค์ เพื่อหาปัจจัยหลักที่เกี่ยวข้องกับการภาวะน้ำหนักเกิน และผลกระทบของภาวะน้ำหนักเกินที่มีต่อสุขภาพของเด็กวัยเรียนของไทย โดยใช้ข้อมูลการสำรวจสุขภาพประชาชนไทยโดยการตรวจร่างกายครั้งที่ 4 ปีพ.ศ. 2551-2552 ผลการศึกษาพบว่ากลุ่มตัวอย่างมากกว่าครึ่งอาศัยอยู่ในเขตเมือง เป็นเพศหญิง และเพศชาย ใกล้เคียงกัน มีอายุเฉลี่ย 9 ปี ผลจากการวิเคราะห์การถดถอยพหุโลจิสติกเชิงอันดับ พบว่า บิดาที่มีระดับการศึกษาสูง อาศัยอยู่ในเขตเทศบาล และ ประกอบอาชีพเกษตรกรรม และบุตรคนเดียวที่อาศัยในเขตเมือง ใช้เวลาในการเรียนจากคอมพิวเตอร์อย่างน้อย 1 ชั่วโมงต่อวัน และ 2 ชั่วโมงต่อวัน มีโอกาสสูงสุดต่อการเกิดภาวะน้ำหนักเกิน สำหรับการวิเคราะห์การถดถอยพหุโลจิสติก พบว่า เด็กที่มีภาวะอ้วน และ น้ำหนักเกิน และ มีอายุอยู่ในช่วงระหว่าง 10 ถึง 12 ปี พบว่า มีโอกาสสูงสุดต่อการเกิดภาวะความดันโลหิตสูง ทั้งบุตรคนเดียวและบุตรที่มีพี่น้องหลายคน ผู้กำหนดนโยบาย หรือ ผู้ที่เกี่ยวข้องจึง ควรพิจารณาคุณลักษณะที่แตกต่างกันของ เด็ก ผู้ปกครอง และ สิ่งแวดล้อม ที่มีต่อการภาวะน้ำหนักเกินที่แตกต่างกันทั้งจากปัจจัยเด็ก บิดามารดา และ สิ่งแวดล้อม ในโครงการควบคุม หรือลดน้ำหนักของเด็กวัยเรียน
