ผลของโปรแกรมการดูแลผิวหนังต่อการหายของแผลในผู้ป่วยที่มีภาวะผิวหนังอักเสบจากการควบคุมการขับถ่ายไม่ได้

Abstract

การศึกษาครั้งนี้เป็นการวิจัยเชิงทดลองแบบสุ่มและมีกลุ่มควบคุม เพื่อศึกษาผลของโปรแกรมการดูแลผิวหนังต่อการหายของแผลในผู้ป่วยที่มีภาวะผิวหนังอักเสบจากการควบคุมการขับถ่ายไม่ได้ กลุ่มตัวอย่างเป็นผู้ป่วยเพศชายและหญิงอายุ 18 ปี ขึ้นไป ที่มีภาวะผิวหนังอักเสบจากการควบคุมการขับถ่ายไม่ได้ ระยะเริ่มแรก - ระยะรุนแรง (mild-severe IAD) แบ่งเป็นกลุ่มควบคุมและกลุ่มทดลอง กลุ่มละ 18 ราย โดยกลุ่มทดลองได้รับโปรแกรมการดูแลผิวหนังผู้ป่วยที่มีภาวะผิวหนังอักเสบจากการควบคุมการขับถ่ายไม่ได้ และกลุ่มควบคุมได้รับการพยาบาลตามปกติ ประเมินภาวะผิวหนังอักเสบจากการควบคุมการขับถ่ายไม่ได้ โดยใช้แบบประเมินระดับความรุนแรงของภาวะผิวหนังอักเสบของ Junkin (2008) และประเมินความก้าวหน้าการหายของภาวะผิวหนังอักเสบโดยแบบประเมินการหายของแผลของ PUSH Tool 3.0 วิเคราะห์ข้อมูลด้วยสถิติบรรยาย เปรียบเทียบความแตกต่างของค่าเฉลี่ยคะแนนความก้าวหน้าการหายของแผลระหว่างกลุ่มหลังได้รับโปรแกรมการดูแลผิวหนังเป็นระยะเวลา 7 วัน ด้วยสถิติ Mann - Whitney U Test และเปรียบเทียบการเปลี่ยนแปลงระดับความรุนแรงของภาวะผิวหนังอักเสบจากการควบคุมการขับถ่ายไม่ได้ด้วยสถิติ Chi Square Test ผลการวิจัยพบว่า กลุ่มที่ได้รับโปรแกรมการดูแลผิวหนังผู้ป่วยที่มีภาวะผิวหนังอักเสบจากการควบคุมการขับถ่ายไม่ได้ มีค่าเฉลี่ยคะแนนความก้าวหน้าการหายของแผลลดลงมากกว่ากลุ่มที่ได้รับการพยาบาลปกติอย่างมีนัยสำคัญทางสถิติ (Z=23.5, p=0.00) และมีระดับความรุนแรงของภาวะผิวหนังอักเสบจากการควบคุมการขับถ่ายไม่ได้ลดลงมากกว่ากลุ่มที่ได้รับการพยาบาลตามปกติ อย่างมีนัยสำคัญทางสถิติ (χ 2 =18.836,p=0.00)

This study is a randomized experimental research with control group to study the effects of skin care protocol for patients with incontinence-associated dermatitis. Male and female patients aged 18 years and over with mild to severe incontinence-associated dermatitis were included in the study. Both control and experimental groups had 18 patients. The experimental group was given the skin care protocol for patients with incontinence-associated dermatitis while the control group received regular nursing procedures. The incontinence-associated dermatitis was assessed by Junkin’s dermatitis severity assessment form (Junkin, 2008). The assessment of wound healing progression was performed by PUSH tool 3.0. The data were analyzed by descriptive statistics. The difference in mean wound healing score between 2 groups after receiving 7-day skin care protocol was compared using the Mann-Whitney U Test. The change in severity degree of incontinence-associated dermatitis was compared using Chi Square Test. The results demonstrated that the group that received the skin care protocol for patients with incontinence-associated dermatitis had lower mean wound healing progression score than the group that received regular nursing procedures, which was statistically significant (Z=23.5,p=0.00). When compared to the group receiving regular nursing procedures, the skin care protocol group demonstrated a statistically significant lower degree of severity (χ 2 =18.836,p=0.00).
