ความพร้อมในการใช้ระบบการแพทย์ทางไกลของผู้ป่วยใน คลินิกอายุรศาสตร์ผู้สูงอายุ โรงพยาบาลศิริราช

Abstract

ประเทศไทยได้เข้าสู่สังคมผู้สูงอายุและพบความชุกของการเกิดโรคเรื้อรังที่สูงขึ้น โดยสถานการณ์การแพร่ระบาดของโรคติดเชื้อไวรัสโคโรนา 2019 เป็นตัวเร่งให้เกิดความก้าวหน้าของระบบการแพทย์ทางไกล เพื่ออำนวยความสะดวกแก่ผู้ป่วยในการนัดหมายพบแพทย์ แจ้งคิวให้บริการ และการติดต่อกับบุคลากรทางการแพทย์ ซึ่งคลินิกอายุรศาสตร์ผู้สูงอายุ โรงพยาบาลศิริราช ได้นำแอปพลิเคชัน “Siriraj Connect” มาใช้ อย่างไรก็ตามการเลือกใช้บริการนี้ยังคงอยู่ในระดับที่ต่ำ และยังไม่เคยประเมินความพร้อมในการใช้งานมาก่อน การศึกษานี้เป็นรูปแบบภาพตัดขวาง มีวัตถุประสงค์เพื่อประเมินความพร้อมในการใช้ระบบการแพทย์ทางไกลของผู้ป่วยในคลินิกอายุรศาสตร์ผู้สูงอายุ โรงพยาบาลศิริราช กลุ่มตัวอย่างที่ศึกษาคือ ผู้ป่วยอายุ 60 ปี ขึ้นไป โดยใช้สถิติเชิงพรรณนาอธิบายคุณลักษณะ และใช้สถิติถดถอยโลจิสติคทวิเพื่อหาปัจจัยที่เกี่ยวข้องกับความพร้อมในการใช้ระบบการแพทย์ทางไกล การศึกษานี้เก็บข้อมูลในกลุ่มตัวอย่างที่เป็นผู้สูงอายุ 84 ราย มีอายุเฉลี่ย 78.36 ปี (SD=7.82) และอายุสูงสุด 97 ปี มีผู้สูงอายุเพียงแค่ร้อยละ 41.7 เท่านั้นที่มีทักษะจำเป็นต่อการใช้ระบบการแพทย์ทางไกล อุปสรรคหลักในการใช้ระบบการแพทย์ทางไกลคือการขาดความคุ้นเคยในการใช้สมาร์ทโฟนหรือแท็บเล็ต ปัจจัยที่มีความสัมพันธ์กับความพร้อมอย่างมีนัยสำคัญทางสถิติ ได้แก่ กลุ่มผู้สูงอายุตอนต้น ระดับการศึกษาสูง สถานะทางการเงินที่ดี และทัศนคติเชิงบวกต่อการใช้ระบบการแพทย์ทางไกล การนำผลของสารนิพนธ์ไปใช้: ในช่วงสถานการณ์การแพร่ระบาดของโรคติดเชื้อไวรัสโคโรนา 2019 ได้มีการแนะนำการใช้ระบบการแพทย์ทางไกลสำหรับผู้ป่วยกลุ่มโรคเรื้อรัง แต่ยังไม่มีการประเมินความพร้อมในการใช้งานของผู้ป่วย อย่างไรก็ตามพบว่าผู้สูงอายุจำนวนมากยังไม่มีความพร้อมสำหรับเทคโนโลยีใหม่ ดังนั้นการประเมินทักษะและการสนับสนุนด้านเทคโนโลยีจึงเป็นกลยุทธ์สำคัญที่จะทำให้เกิดความมั่นใจถึงประโยชน์ของการใช้ระบบการแพทย์ทางไกลในผู้ป่วยสูงอายุ

Thailand has emerged as an aging society with a high prevalence of chronic diseases. The onset of the COVID-19 pandemic acted as a catalyst for the advancement of telemedicine. To facilitate patient access to appointments, service queues, and communication with medical staff, the Geriatrics Clinic at Siriraj Hospital introduced the "Siriraj Connect" application. However, the utilization of this service remains low, and its readiness has never been evaluated. This cross-sectional study aimed to assess the readiness of elderly patients at the geriatric clinic in Siriraj Hospital to use telemedicine. A constructed questionnaire was administered to participants aged 60 years and older, and data were characterized using descriptive statistics. Binary logistic regression was employed to identify factors associated with the readiness to use telemedicine. The study included 84 elderly participants, with an average age of 78.36 (SD=7.82) years and a maximum age of 97 years. Only 41.7% of the elderly possessed the necessary skills to utilize telemedicine. The primary obstacle to using telemedicine was their lack of familiarity with smartphones or tablets. Factors significantly associated with readiness to use telemedicine were younger age, higher education level, favorable financial status, and positive perception of telemedicine. Implication of thematic paper: During the COVID-19 pandemic, telemedicine was recommended for most chronic disease patients without the evaluation for readiness of telemedicine. However, many elderly individuals were not ready for new technology. The skill evaluation and technology support will be the important strategies to ensure the benefit of telemedicine to elderly patients.
