Abstract
Optical surface imaging (OSI) system is currently introduced for setup verification, intrafractionation monitoring as well as for gating radiotherapy. This system is found to be an attractive IGRT for deep inspiration breath hold (DIBH) technique in left-sided breast cancer patients. The purpose of this study was to evaluate the performance and reliability of the CatalystTM OSI system which is installed at Division of Radiation Oncology, Siriraj Hospital for image guidance in DIBH application. Performance of the OSI in the topic of system communication, static and dynamic accuracy, as well as reproducibility of the detection system, a comprehensive acceptance and commissioning tests were performed according to AAPM TG-147 guideline. Then, the overall accuracy of treatment process in OSI implementation was examined using the end-to-end test with thorax phantom. For patient positioning, a total of 253 individual setups from the OSI in 20 patients were retrospectively compared with the routine IGRTs at the center. The couch deviations from 83-image CBCT and 170 images of kV planar imaging after patient positioning with OSI were recorded and analyzed. Our investigation illustrated the OSI were properly installed and accurately calibrated to the CT simulator and LINAC. The communication and integration of the OSI with peripheral equipment were fully functioning. The system was able to detect the interested object accurately within 1 mm for translational direction and 1 degree for rotational directions. For dynamic movement, the errors of detection system was not found when it detected the object at isocenter and up to 10 cm from the center. The difference between setting and reading amplitude less than 1 mm was noticed at the distance 15 to 25 cm from isocenter, while the maximum variation (within 2mm) was observed when the distance grew up to 25 cm from the center. The whole treatment process with the OSI depicted properly integration and agreement within 2 mm. Moreover, the overall setup accuracy with the OSI when compared with two different reference surface images (SentinelTM and TPS body contour) showed similar results. The retrospective comparison using the CatalystTM OSI for patient positioning presented the median deviations in lateral, longitudinal, and vertical axis for CBCT were 1.8 mm, 2.1 mm, and 1.7 mm, respectively. Comparison with kV planar imaging, the median deviations were shown at 1.7 mm in lateral, 3.1 mm in longitudinal, and 1.7 mm in vertical directions. In our study, the data presented the quite similar results in lateral and vertical directions for both CBCT and kV planar images, but the higher deviation was found in longitudinal axis with statistically significant (p=0.038) for kV planar images. In summary, the CatalystTM OSI system is an efficient image guidance for patient setup and accurate for implementing in DIBH technique.
ระบบภาพพื้นผิวผู้ป่วยที่จัดเก็บด้วยเทคโนโลยีแสง (Optical Surface Imaging; OSI) ใช้สำหรับการจัดท่าผู้ป่วย และตรวจจับการเคลื่อนที่ระหว่างการฉายรังสี รวมถึงใช้ในการรักษาแบบ Gating ปัจจุบันได้ถูกนำมาประยุกต์ใช้ในผู้ป่วยมะเร็งเต้านมด้านซ้ายที่รักษาด้วยเทคนิค Deep Inspiration Breath Hold (DIBH) การศึกษานี้เป็นการประเมินประสิทธิภาพการทำงานของระบบ OSI และตรวจสอบความถูกต้องในการจัดท่าผู้ป่วยมะเร็งเต้านมด้านซ้ายที่รักษาด้วยเทคนิค DIBH โดยตรวจสอบการทำงานของระบบ OSI ตามคำแนะนำของ AAPM TG-147 ในการเชื่อมต่อและส่งข้อมูลกับเครื่องมือและระบบที่เกี่ยวข้อง ศึกษาความถูกต้องของระบบการตรวจจับพื้นผิวที่อยู่นิ่งด้วยการใช้หุ่นจำลอง และพื้นผิวที่มีการเคลื่อนไหวแบบต่อเนื่อง ศึกษาความคงที่ของระบบในการตรวจจับพื้นผิว ตรวจสอบความถูกต้องของกระบวนการนำ OSI ในการจัดท่าผู้ป่วยด้วยการใช้หุ่นจำลองทรวงอกมนุษย์ ทำการศึกษาแบบย้อนหลังและรวบรวมข้อมูลความคลาดเคลื่อนของตำแหน่งการฉายรังสีจากการใช้ระบบ OSI ในการจัดท่าผู้ป่วยจำนวน 20 ราย มาเปรียบเทียบและวิเคราะห์กับข้อมูลที่ได้จากภาพถ่ายเอกซเรย์ 3 มิติชนิด Cone Beam Computed Tomography (CBCT) จำนวน 83 ภาพและภาพถ่ายเอกซเรย์ 2 มิติระดับพลังงาน kilovoltage (kV) จำนวน 170 ภาพ ผลการศึกษาพบว่าระบบ OSI มีการติดตั้งและทำงานได้อย่างมีประสิทธิภาพ โดยระบบสามารถส่งข้อมูลผู้ป่วยไปยังเครื่องมือและอุปกรณ์ที่ทำงานร่วมกันได้อย่างถูกต้อง ระบบมีความแม่นยำในการตรวจจับภาพพื้นผิวที่ความถูกต้องภายใน 1 มิลลิเมตร สำหรับแนว Translation และ 1 องศา สำหรับแนว Rotation สำหรับการตรวจจับพื้นผิวหุ่นจำลองที่มีการเคลื่อนไหวแบบต่อเนื่องพบว่าระบบ OSI ไม่แสดงค่าความคลาดเคลื่อนในการตรวจจับวัตถุเมื่อหุ่นจำลองอยู่ภายในระยะทาง 10 เซนติเมตรจากตำแหน่ง Isocenter โดยระบบเริ่มมีความคลาดเคลื่อนในการตรวจจับเมื่อตำแหน่งหุ่นจำลองอยู่ห่างจาก Isocenter ที่ระยะทาง 15 ถึง 25 เซนติเมตร และพบความคลาดเคลื่อนที่มากที่สุด 2 มิลลิเมตร เมื่อหุ่นจำลองอยู่ที่ระยะ 25 เซนติเมตร สำหรับความถูกต้องของกระบวนการนำระบบ OSI มาใช้ในการตรวจจับตำแหน่งของหุ่นจำลองมีค่าความคลาดเคลื่อนน้อยกว่า 2 มิลลิเมตรในทุกทิศทาง นอกจากนั้นการทดลองใช้ภาพพื้นผิวอ้างอิงจาก SentinelTM และจากภาพ CT พบว่าให้ความถูกต้องในการตรวจสอบตำแหน่งที่ใกล้เคียงกัน การศึกษาย้อนหลังในผู้ป่วยมะเร็งเต้านมด้านซ้ายเมื่อใช้ระบบ OSI เทียบกับภาพ CBCT พบค่ากลางของข้อมูลในแนว Lateral, Longitudinal และ Vertical เท่ากับ 1.8 มม, 2.1 มม, และ 1.7 มม ตามลำดับ และเมื่อเปรียบเทียบกับภาพถ่าย kV มีค่ากลางของข้อมูลในแนว Lateral ,Longitudinal และ Vertical เท่ากับ 1.7 มม, 3.1 มม, และ 1.7 มม ตามลำดับ ในการศึกษานี้พบว่าค่าความ คลาดเคลื่อนในแนว Lateral และ Vertical เมื่อเปรียบเทียบกับภาพทั้งสองระบบให้ผลที่ใกล้เคียงกัน แต่ค่าความคลาดเคลื่อนในแนว Longitudinal ในภาพ kV มีค่ามากกว่าภาพ CBCT อย่างมีนัยสำคัญทางสถิติ (p=0.038) จากผลการศึกษาจึงสามารถสรุปได้ว่าระบบ CatalystTM OSI มีประสิทธิภาพในการตรวจสอบตำแหน่งก่อนการรักษาและมีความถูกต้องในการนำไปใช้ตรวจจับพื้นผิวผู้ป่วยในเทคนิค DIBH สำหรับผู้ป่วยมะเร็งเต้านมด้านซ้าย
