Abstract
Several lines of evidence have suggested that certain types of human -papillomavirus (HPV) and herpes simplex virus type 2 (HSV-2) are associated with cervical carcinoma. To study for such association in Thai women, determination for the presence of HPV genome and HSV-2 Bgl II N fragment was performed in cervical tissues derived from patients with premalignant and malignant lesions. By non-isotopic in situ hybridization, 50% of samples were positive for HPV, type 16/18 were more commonly observed than others. While 15% of histologically-proved normal cervical tissues randomly taken from the patients were positive, all cervical tissues derived from non-cervical cancer subjects were negative for HPV. The presence of HSV-2 Bgl II N sequence detected by nested PCR in HPV-positive and negative groups was similar, i.e., approximately 20 % and 18%, respectively. The results suggested that other cofactor(s) may also interact with HPV and/or HSV-2 that eventually lead to malignancy conversion. Another factor which may play an important role in cervical carcinogenesis is the tumor suppressor gene-p53. In vitro studies have shown that E6 product from oncogenic type of HPV could bind to p53 product and finally led to p53 degradation through the ubiquitin protease system. Thus, in this study the status of p53 was also examined in cervical cancer tissue samples with HPV positive and HPV negative by immunohistochemistry technique. Only one of 52 samples was positive for p53; this p53 positive case had histological diagnosis of CIN III and was positive with all HPV probe groups as well. Further molecular analysis is needed to indicate whether p53 mutation plays role in cervical carcinoma in these patients.
ข้อมูลจากการศึกษาวิจัยในแง่มุมต่างๆ บ่งชี้ว่า human papillomavirus (HPV) บาง types และ herpes simplex virus type 2 (HSV-2) มีความสัมพันธ์กับการก่อให้เกิดมะเร็ง ปากมดลูก การศึกษาครั้งนี้จึงได้ศึกษาหาความสัมพันธ์ระหว่าง เชื้อไวรัสทั้งสองชนิดกับการเกิดมะเร็งปากมดลูกในหญิงไทย โดยได้นำชิ้นเนื้อมะเร็งในระยะต่างๆ มาทำการตรวจหายีน ของ HPV และดีเอ็นเอที่มีส่วน transforming gene ของ HSV-2 คือ Bgl II N fragment เมื่อใช้เทคนิค non-isotopic in situ hybridization สามารถตรวจพบยีนของ HPV ได้ ประมาณ 50% ส่วนใหญ่เป็น type 16/18 และพบยีนของ HPV ในชิ้นเนื้อปกติของผู้ป่วยมะเร็งปากมดลูกได้ร้อยละ 15 ในขณะที่ ไม่พบเมื่อตรวจชิ้นเนื้อที่ได้จากผู้ป่วยอื่นๆ ซึ่งไม่ได้เป็นมะเร็ง ปากมดลูก ส่วน HSV-2 Bgl II N fragment ซึ่งตรวจหา โดยวิธี nested PCR สามารถพบได้ประมาณร้อยละ 20 ใน กลุ่มผู้ป่วยที่ตรวจพบ HPV และร้อยละ 18 ในกลุ่มที่ตรวจไม่พบ HPV แต่ไม่พบเลยเมื่อตรวจชิ้นเนื้อที่ได้จากผู้ป่วยที่ไม่ได้เป็น มะเร็งปากมดลูกหรือเนื้อเยื่อปกติของผู้ป่วยมะเร็งปากมดลูก ดังนั้นจึงเป็นไปได้ที่อาจมีปัจจัยอื่นๆ อีกที่เข้ามามีบทบาท ร่วมกับ HPV และ/หรือ HSV-2 ในการก่อให้เกิดมะเร็ง ปากมดลูก ปัจจัยอื่นๆ ที่อาจมีบทบาทในการก่อให้เกิดมะเร็ง ปากมดลูกซึ่งรวมอยู่ในการศึกษานี้ด้วยคือ ยีนต้านมะเร็ง -p53 ซึ่งการศึกษาในหลอดทดลองพบว่า ผลผลิตของยีน E6 ของ HPV ที่เป็น oncogenic type สามารถจับกับ โมเลกุลของ p53 และนำไปสู่การย่อยสลายโดยระบบ ubiquitin ในที่สุด ดังนั้นในการศึกษาครั้งนี้ จึงได้ทำ การหาผลผลิตของ p53 จากชิ้นเนื้อมะเร็งที่มีการตรวจพบ HPV DNA และที่ตรวจไม่พบ HPV โดยเทคนิค Immunohistochemistry จากตัวอย่างชิ้นเนื้อจำนวน 52 ราย พบผลบวก เพียงหนึ่งราย ซึ่งเป็นผู้ป่วย CIN III ที่มีผลบวกต่อ HPV probe ทุกชุด จากข้อมูลที่ได้จึงยังไม่สามารถสรุปถึงบทบาท ของ p53 ในมะเร็งปากมดลูกได้ และการศึกษาให้ทราบถึง บทบาทที่แน่นอนของ p53 ควรที่จะได้นำเทคนิควิเคราะห์ใน ระดับอณูเข้าร่วมด้วย
