ICD-10-TM ontology development in infectious disease for support coding system

Abstract

Medical records are records of the patient's illness history and information about the patient's care. However, ICD-10 coding is another important for evaluating the quality of medical records. ICD-10 coding guidelines are numerous and extremely complex. This can cause errors or incomplete ICD-10 codes. This study aimed to analyze ICD-10 coding criteria in infectious diseases by developing a prototype ontology and a semantic retrieval system to support decision-making in choosing the correct and appropriate disease code. By creating an ontology and developing a semantic retrieval system based on a physician's diagnosis and coding criteria in infectious diseases for users to choose an ICD-10 code correctly according to the guidelines and allow users to evaluate the suitability of the information search system that has been created. The results found that the overall user satisfaction of the system was at the strongly agree level (x̅ = 4.70, SD. = 0.74) due to the overall use of the system is complete content information covering the needs of users, has an uncomplicated and easy to use. Further research can be developed more than the prototype ontology and semantic retrieval system for ICD-10 coding guidelines of other diseases to support decision-making in collecting ICD-10 codes correctly. IMPLICATION OF THEMATIC PAPER: An ontology and the semantic retrieval system of ICD-10 coding, it can be a tool to help these medical coders find ICD-10 coding guidelines to help them decide on the right and appropriate ICD-10 coding, this results in the quality of medical records that are accurate according to the National Health Security Office (NHSO) guidelines.

การบันทึกเวชระเบียนเป็นการบันทึกประวัติการเจ็บป่วยและข้อมูลในการดูแลรักษาของผู้ป่วย ดังนั้นจึงมีการตรวจสอบคุณภาพการบันทึกเวชระเบียน รหัสทางการแพทย์ถือเป็นอีกหนึ่งสิ่งสำคัญสำหรับการประเมินคุณภาพการบันทึกเวชระเบียน อย่างไรก็ตาม หลักเกณฑ์การให้รหัส ICD-10 มีความซับซ้อนเป็นอย่างมาก และยังเป็นการสืบค้นหลักเกณฑ์ต่าง ๆ จากตำราหลายหลากหลายแหล่งที่มา ทำให้เกิดปัญหาการให้รหัส ICD-10 ที่ผิดพลาด งานวิจัยนี้มีวัตถุประสงค์เพื่อวิเคราะห์หลักเกณฑ์การให้รหัส ICD-10 ในกลุ่มโรคติดเชื้อ โดยได้ทำการพัฒนาต้นแบบออนโทโลยี และระบบสืบค้นข้อมูลเพื่อช่วยสนับสนุนการตัดสินใจในการเลือกรหัสโรคได้อย่างถูกต้องและเหมาะสม โดยงานวิจัยนี้ได้ทำการสร้างออนโทโลยี เชื่อมความสัมพันธ์ของหลักเกณฑ์การให้รหัส ICD-10 ในกลุ่มโรคติดเชื้อ และนำมาพัฒนาระบบค้นคืนสารสนเทศเชิงความหมาย เพื่อการสืบค้นหลักเกณฑ์การให้รหัส ICD-10 ตามคำวินิจฉัยของแพทย์ และเกณฑ์การให้รหัสในกลุ่มโรคติดเชื้อ เพื่อให้ผู้ใช้งานได้ทำการเลือกรหัส ICD-10 ได้อย่างถูกต้องและเหมาะสมตามหลักเกณฑ์ของสำนักงานหลักประกันสุขภาพแห่งชาติ และให้ผู้ใช้งานทำการประเมินความเหมาะสมของระบบสืบค้นข้อมูลที่ได้จัดทำขึ้น ผลการศึกษา พบว่า ผู้ใช้งานระบบมีความพึงพอใจโดยรวมอยู่ในระดับพึงพอใจมากที่สุด (ค่าเฉลี่ย = 4.70 ส่วนเบี่ยงเบนมาตรฐาน = 0.74) เนื่องจากการใช้งานภาพรวมของระบบมีข้อมูลเนื้อหาครบถ้วนครอบคลุมตามที่ผู้ใช้งานต้องการ มีการใช้งานที่ไม่ซับซ้อน ง่ายต่อการใช้งาน ในอนาคตสามารถพัฒนางานวิจัยเพิ่มเติมเกี่ยวกับการพัฒนาออนโทโลยีต้นแบบและระบบสืบค้นข้อมูลหลักเกณฑ์การให้รหัส ICD-10 ในกลุ่มโรคอื่น ๆ เพื่อนำไปใช้สนับสนุนการตัดสินใจในการเลือกรหัสโรคได้อย่างถูกต้องและเหมาะสม การนำผลของสารนิพนธ์ไปใช้: งานวิจัยนี้ได้ทำการพัฒนาออนโทโลยีต้นแบบและระบบสืบค้นข้อมูลหลักเกณฑ์การให้รหัส ICD-10 ในโรคติดเชื้อ เพื่อช่วยสนับสนุนการตัดสินใจเลือกรหัส ICD-10 ที่ถูกต้องและเหมาะสม ส่งผลให้มีคุณภาพในการบันทึกเวชระเบียนที่ถูกต้องตามหลักเกณฑ์ของสำนักงานหลักประกันสุขภาพแห่งชาติ
