ความเป็นธรรมของการให้บริการกายภาพบำบัดผู้ป่วยโรคหลอดเลือดสมอง จังหวัดนครนายก

Abstract

การวิจัยนี้เป็นการวิจัยเชิงสำรวจแบบภาคตัดขวาง เพื่อศึกษาความเป็นธรรมของการ ให้บริการทางกายภาพบำบัด ในผู้ป่วยโรคหลอดเลือดสมอง จังหวัดนครนายก จำนวน 137 ราย โดย ใช้แบบสอบถามและการสัมภาษณ์ ผู้ป่วยโรคหลอดเลือดสมองหรือผู้ดูแล ตั้งแต่เดือน มิถุนายน ถึง ธันวาคม 2554 การวิเคราะห์ข้อมูลประกอบด้วยการแจกแจงความถี่ ร้อยละ และสถิติไคสแควร์ ผลการวิจัย พบว่า การให้บริการกายภาพบำบัดของผู้ป่วยโรคหลอดเลือดสมอง ใน จังหวัดนครนายก ไม่มีความเป็นธรรมในแนวราบทั้ง 3 มิติ (มิติด้านชนิดของการให้บริการ มิติด้าน เวลา และ มิติของความถี่ในการรักษาทางกายภาพบำบัด) โดยพิจารณาจากความสัมพันธ์ที่มี นัยสำคัญทางสถิติ ระหว่าง ระดับความเจ็บป่วย กับ 1) จำนวนของชนิดการรักษาทางกายภาพบำบัด ( p=0.001) และ 2) ระยะเวลาและความถี่ในการรักษาทางกายภาพบำบัด (p=0.045) และพบปัจจัยที่มี ความสัมพันธ์ที่มีนัยสำคัญทางสถิติกับ การให้บริการกายภาพบำบัด คือ อายุ (p=0.016) ระดับ การศึกษา (p=0.046) ที่อยู่อาศัย (p=0.023) รายจ่ายของผู้รับบริการ (p=0.002) สิทธิการรักษาพยาบาล (p<0.001) และผู้ดูแลหลักของผู้รับบริการ (p= 0.023) เพื่อให้เกิดความเป็นธรรมในการให้บริการกายภาพบำบัดในผู้ป่วยโรคหลอดเลือด สมอง จากผลการวิจัย ได้มีข้อเสนอแนะสำหรับผู้บริหารคือ การนำผลการวิจัยนี้เป็นข้อมูล ประกอบการจัดทำแผนปฏิบัติงาน หรือ พิจารณานำชนิดของการรักษา ปริมาณความถี่ของการรักษา และระยะเวลาในการรักษาทางกายภาพบำบัด ในการจัดทำคู่มือมาตรฐานในการดูแลรักษาผู้ป่วยโรค หลอดเลือดสมอง

This research was a cross-sectional survey research to study equity of physical therapy services for 137 patients with stroke in Nakhonnayok Province, Thailand using questionnaires and interviews from June to December 2011. The analyses consisted of frequency distributions, percentage and chi-square. The results show that physical therapy services for stroke patients at Nakhonnayok province showed horizontal non-equivalence in all three-dimensions of physical therapy (number of treatment, time for treatment and frequency of treatment). There were statistically significant relationship between the level of illness and 1) number of physical therapy types (p=0.001) and time and frequency of physical therapy (p=0.045). The factors that had statistically significant relationship with physical therapy services were age (p=0.016), education (p=0.046), residences of patients (p=0.023), expense of patients (p=0.002), health care scheme (p <0.001) and Primary the main caregiver of patients (p=0.023). To achieve equity for physical therapy among stroke patients, policy makers are recommended to use findings from this study and consider threedimensions of physical therapy for activity plan or development of standard practice guideline on physical therapy for patients with stroke.
