The effect of triclosan/copolymer containing dentifrice during periodontal maintenance in type 2 diabetic patients

Abstract

Several studies have shown that diabetes mellitus increased the risk of developing periodontitis. Recent evidence suggests that hyperglycemia may induce inflammatory cytokine productions, which have been implicated in the pathogenesis of periodontal destruction. Clinical studies have indicated that diabetes may adversely affect and impede healing following periodontal therapy. Self-performed mechanical plaque control and periodontal maintenance are the important means to control periodontal disease. Also, several chemical plaque control agents have been proposed to help maintain the periodontal status. The objective of this study is to evaluate the effect of a triclosan/copolymer/fluoride dentifrice during maintenance period following non-surgical periodontal therapy in type 2 diabetic patients. Fifty diabetics with periodontitis volunteered to participate in this study. At baseline, the participants received non-surgical periodontal therapy and oral hygiene instruction followed by periodontal maintenance care. At month 3, the subjects were randomly assigned to use a triclosan/copolymer/fluoride (24 subjects) or a standard fluoride (24 subjects) dentifrice. Periodontal parameters (PAI, BOP, PD, CAL), metabolic status (HbA1c, FPG) and the levels of inflammatory mediators (IL-1ß, PGE2) were recorded at baseline, month 3 and month 6. Subjects using the triclosan/copolymer/fluoride dentifrice exhibited improved clinical attachment level and maintained low level of inflammatory mediators and depth of periodontal pocket compared to those using the standard fluoride dentifrice over the 3-month maintenance interval. Whereas, both groups showed no significant change in metabolic status. The results suggest that an oral hygiene regimen including a triclosan/copolymer/fluoride dentifrice significantly improve and maintain periodontal status in diabetics, but it has no effect on metabolic status of diabetes mellitus.

การศึกษาจำนวนมากแสดงให้เห็นว่าผู้ป่วยโรคเบาหวานมีความเสียงต่อการเกิดโรคปริทันต์อักเสบเพิ่มขึ้น สภาวะที่น้ำตาลในเลือดสูงชักนำให้เกิดการสร้างสารสื่อกลางการอักเสบซึ่งเกี่ยวข้องกับขบวนการเกิดโรคปริทันต์และขัดขวางการหายหลังการรักษาโรคปริทันต์ งานวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาผลของการใช้ยาสีฟันที่มีส่วนผสมของไตรโคลซาน/โคโพลิเมอร์ระหว่างการคงสภาพปริทันต์ในผู้ป่วยเบาหวานผู้ป่วย 50 คนเป็นโรคปริทันต์อักเสบร่วมกับโรคเบาหวานชนิดที่ 2 เมื่อเริ่มต้นผู้ป่วยทุกคนใช้เวลาประมาณ 3 เดือนรับการรักษาโรคปริทันต์อักเสบโดยไม่ใช้วิธีผ่าตัด และคำแนะนำในการทำความสะอาดช่องปากที่เหมาะสม หลังจากนั้นทำการสุ่มแบ่งผู้ป่วยเป็น 2 กลุ่ม กลุ่มแรกได้รับยาสีฟันที่มีส่วนผสมของไตรโคลซาน/โคโพลิเมอร์ ส่วนกลุ่มควบคุมได้รับยาสีฟันที่มีส่วนผสมหลักของฟลูโอไรด์ตลอด 3 เดือนของการคงสภาพปริทันต์ มีการบันทึกค่าของตัวแปรโรคปริทันต์อักเสบ(PAI, BOP, PD,CAL) ค่าของระดับน้ำตาลในเลือด(HbAlc, FPG) และค่าของระดับสารสื่อกลางการอักเสบ(IL-1B, PGE2) เมื่อเริ่มต้นการรักษา เดือนที่ 3 และ เดือนที่ 6 ผลทางคลินิกพบว่ากลุ่มทดสอบมีค่าระดับการยึดเกาะของเนื้อเยื่อยึดต่อ(CAL)ลดลง และ สามารถคงสภาพค่าของระดับสารสื่อกลางการอักเสบ และความลึกของร่องปริทันต์เมื่อเปรียบเทียบ กับกลุ่มควบคุม แต่ทั้งสองกลุ่มไม่มีการเปลี่ยนแปลงค่าของระดับน้ำตาลในเลือดทั้งสองค่า โดยสรุปพบว่า การใช้ยาสีฟันที่มีส่วนผสมของไตรโคลซาน/โคโพลิเมอร์ระหว่างคงสภาพปริทันต์หลังการรักษาโรคปริทันต์โดยไม่ใช้วิธีผ่าตัดสามารถคงสภาวะปริทันต์ที่ดีหลังการรักษาไว้ได้ แต่ไม่มีผลต่อการเปลี่ยนแปลงค่าของระดับน้ำตาลในเลือดในผู้ป่วยเบาหวานชนิดที่ 2
