The stability of ethanol in blood under experimental conditions

Abstract

At present the number of blood samples for analysis of ethanol is greatly increasing and the question of the stability of ethanol in the specimen is a practical problem. Ethanol is volatile substance and easily changed in the course of time and stored condition when performing analysis in laboratory. Therefore, the study was designed and set up in order to do analysis of ethanol in blood samples, comparing temperature and time intervals for stored blood from fifty autopsy cases at the forensic medicine department. Blood samples collected from the cadavers with 1% sodium fluoride as preservative were analyzed immediately for 100% ethanol concentration by Headspace Gas Chromatography technique. Remaining blood samples was transferred to 3 ml polypropylene tubes for storage at room temperature, 4 °C and -20°C. The storage samples were analyzed for ethanol at; 2, 4, 7, 14, 28, 60 and 90 days of storage time interval. It was found that maximum stability of ethanol was found in stored blood at 4°C and concentration in stored blood at room temperature showed maximum decrease when compare with storage blood at other temperature of the same storage time interval (p<0.01). Ethanol concentration in stored blood at -20℃ was decreased when stored up to 14 days (p<0.05), and concentration in stored blood at 4 ℃ was stable for 28 days. The last study was to compare ethanol concentration and glucose level in blood sample with high (>200 mg/dl) and normal (70-110 mg/dl) glucose amount for the stability of ethanol analysis when stored at room temperature. It was found that ethanol was increased when stored up to 7 days (p<0.01), while glucose was decreased when stored at the same time interval (p<0.01).

ปัจจุบันการตรวจวัดระดับเอทานอลในเลือด เป็นการตรวจที่สำคัญมากทางนิติเวช ศาสตร์ และมีแนวโน้มที่จะเพิ่มมากขึ้นเรื่อยๆ ปัญหาสำคัญของการตรวจวัดคือความถูกต้องในการตรวจ เนื่องจากแอลกอฮอล์เป็นที่สารระเหยได้ง่าย จึงมีปัญหาในการเก็บรักษาสิ่งส่งตรวจ ดังนั้นในการศึกษาครั้งนี้ จึงมีจุดประสงค์เพื่อตรวจวัดระดับของ เอทานอล ในเลือด เมื่อเก็บรักษาไว้ที่อุณหภูมิ และ ระยะเวลาต่างๆ กันโดยศึกษาตัวอย่างเลือด 50 ราย และใช้ 1 เปอร์เซนต์ โซเดียมฟลูโอไรด์ เป็นสารรักษาสภาพ และตรวจวัดระดับของเอทานอลทันที จากนั้นเก็บไว้ 3 อุณหภูมิคือที่อุณหภูมิห้อง, ตู้เย็น (4°C) และช่องแช่แข็ง (-20°C) และตรวจวัดระดับของเอทานอล ทุกวันที่ 2, 4, 7, 14, 28, 60 และ 90 วัน ตามลำดับ จากการทดลองพบว่า ผลการตรวจวัด เมื่อเก็บไว้ในตู้เย็น 4 °C มีความคงตัวดีที่สุดนาน 28 วันในขณะที่การเก็บที่อุณหภูมิ -20°C จะเก็บได้นาน 14 วัน และการ เก็บที่ อุณหภูมิห้องระดับของเอทานอลลดลงและต่างจากกลุ่มอื่นอย่างมีนัยสำคัญเมื่อเทียบในระยะเวลาการเก็บรักษาเท่ากัน (p<0.01) นอกจากนี้ยังทำการศึกษาผลของระดับกลูโคสในเลือดที่มีระดับสูง (>200mg/dl) และ ระดับปกติ (70-110 mg/dl) ต่อปริมาณเอทานอลเมื่อเก็บรักษาไว้ที่อุณหภูมิห้อง และไม่มีการใช้สารรักษาสภาพ พบว่า เอทานอลจะเพิ่มขึ้นสูงสุดเมื่อเก็บไว้นาน 7 วัน ในทั้งสองกลุ่มที่มีระดับกลูโคสสูง (p<0.01) และกลุ่มที่ระดับกลูโคสปกติ (p<0.05) แต่ระดับเอทานอลในกลุ่มที่มีกลูโคสสูงมีระดับเอทานอลสูงกว่ากลุ่มที่มีระดับกลูโคสปกติส่วนการลดลงของกลูโคส จะลดลงมากเมื่อเก็บไว้นาน 7 วัน ในทั้ง 2 กลุ่ม (p<0.01)
