ผลของการประคบเต้านมด้วยผ้าอุ่นชื้นต่อระยะเวลาการเริ่มไหลของน้ำนมในมารดาหลังผ่าตัดคลอดบุตรคนแรก

Abstract

การศึกษาครั้งนี้เป็นการวิจัยชนิดสุ่มแบบมีกลุ่มเปรียบเทียบ ศึกษาประสิทธิผลของการประคบเต้านมด้วยผ้าอุ่นชื้นต่อระยะเวลาการเริ่มไหลของน้ำนมในมารดาหลังผ่าตัดคลอดบุตรคนแรก ที่หอผู้ป่วยหลังคลอด โรงพยาบาลศิริราช คัดเลือกกลุ่มตัวอย่างตามเกณฑ์การคัดเข้า คือ ตั้งใจเลี้ยงลูกด้วยนมแม่ ดัชนีมวลกายก่อนตั้งครรภ์ปกติ เต้านมและหัวนมปกติ ไม่มีโรคที่ส่งผลกระทบต่อการสร้างและหลั่งน้ำนม ไม่มีภาวะหรือโรคที่เป็นข้อห้ามในการบำบัดด้วยความร้อน ไม่สูบบุหรี่หรือใช้สารเสพติด ทารกมีสุขภาพแข็งแรง จำนวน 60 คู่ สุ่มกลุ่มตัวอย่างแบบบล็อกครั้งละ 4 คู่ จนได้กลุ่มทดลองครบจำนวน 30 คู่ และกลุ่มควบคุมจำนวน 30 คู่ เก็บข้อมูลระหว่างวันที่ 7 กรกฎาคม ถึง 10 กันยายน 2564 กลุ่มทดลองได้รับการพยาบาลตามปกติร่วมกับการประคบเต้านมด้วยผ้าอุ่นชื้นที่ผู้วิจัยประดิษฐ์ขึ้น กลุ่มควบคุมได้รับการพยาบาลตามปกติ กลุ่มตัวอย่างทั้งสองกลุ่มได้รับการประเมินการไหลของน้ำนมทุก 2-3 ชั่วโมง หรือเมื่อมารดาแจ้งว่าน้ำนมมา วิเคราะห์ข้อมูลโดยใช้สถิติทดสอบที ผลการศึกษาพบว่ากลุ่มทดลองมีค่าเฉลี่ยระยะเวลาการเริ่มไหลของน้ำนมเท่ากับ 30.43 ชั่วโมง กลุ่มควบคุมมีค่าเฉลี่ยระยะเวลาการเริ่มไหลของน้ำนมเท่ากับ 40.35 ชั่วโมง เมื่อทดสอบความแตกต่างของค่าเฉลี่ยระยะเวลาที่น้ำนมเริ่มไหล พบว่ากลุ่มตัวอย่างในกลุ่มทดลองน้ำนมเริ่มไหลเร็วกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ (t = -5.485; p < 0.001) จากผลการศึกษา มีข้อเสนอแนะว่า การประคบเต้านมด้วยผ้าอุ่นชื้นในมารดาหลังผ่าตัดคลอดบุตรคนแรกช่วยกระตุ้นให้น้ำนมมาเร็วขึ้น แต่ควรศึกษาเพิ่มเติมในมารดาหลังคลอดกลุ่มอื่น เช่น มารดาที่มีความวิตกกังวลว่าน้ำนมมาช้า เป็นต้น

A randomized controlled trial aimed to study the effects of breast compression using warm moist cloth on the first secretion of breast milk in cesarean mothers with the first-born child in the postpartum unit, Siriraj Hospital. Inclusion criteria were having intention to breastfeed, normal pre-pregnancy BMI, normal breast and nipple, no disease related to milk production and milk secretion, no contraindication of heat therapy, no smoking and drug abuse, and the child was healthy. Participants were 60 mother-child dyads. They were assigned to the experimental and the control groups by block randomization, each comprising 30 dyads. Data were collected from July 7 to September 10, 2021. The experimental group received warm moist cloth compression along with routine nursing care, while the control group received only routine nursing care. Both groups expressed breast milk every 2-3 hours or upon perception of milk coming. An independent t-test was used to analyze the data. Results showed that the experimental group had a mean time of the first secretion of breast milk of 30.43 hours after giving birth, while the control group had 40.35 hours. Therefore, the mean time of onset of milk secretion in the experimental group was significantly earlier than that of the control group (t = -5.485; p < 0.001). The results suggested that compression of the breast with warm moist cloth can stimulate the secretion of breast milk among primiparous postpartum mothers with cesarean section. However, it should be studied in other groups of mothers, such as mothers who are concerned about delayed onset of lactogenesis.
