ระยะเวลาการนอนโรงพยาบาลของผู้ป่วยอุบัติเหตุจราจรในโรงพยาบาลพระมงกุฎเกล้า

Abstract

การวิจัยแบบการศึกษาย้อนหลังนี้มีวัตถุประสงค์เพื่อวิเคราะห์ระยะเวลานอนโรงพยาบาล (Hospital length of stay: HLOS) และทดสอบคุณลักษณะของผู้ป่วย และปัจจัยการบริการที่มีความสัมพันธ์กับ HLOS ในผู้ป่วยอุบัติเหตุจราจรที่รักษาในโรงพยาบาลพระมงกุฎเกล้า ใช้ข้อมูลจากฐานข้อมูลอุบัติเหตุของโรงพยาบาล ระหว่าง เมษายน ถึง กันยายน ปี พ.ศ. 2562 โดยแบบบันทึกข้อมูล กลุ่มตัวอย่างเป็นผู้ป่วยบาดเจ็บจากอุบัติเหตุจราจรที่มารักษาที่ห้องฉุกเฉินและรักษาเป็นผู้ป่วยใน จำนวน 139 คน ระยะเวลานอนโรงพยาบาล นับเป็นจำนวนวัน วิเคราะห์ข้อมูลโดยใช้สถิติพรรณนา และสถิติวิเคราะห์ความแตกต่างค่าเฉลี่ย ใช้สถิติ Mann-Whitney U test และ Kruskal-Wallis test ผลการวิจัย พบว่า ผู้ป่วยอุบัติเหตุจราจรจำนวน 139 คน เพศชาย ร้อยละ 89.2 มัธยฐานอายุ 30 ปี (Q1-Q3 =24-48 ปี) ความรุนแรงระดับ Moderate ร้อยละ 43.9 โอกาสรอดชีวิตระดับร้อยละ75-100 ร้อยละ 98.6 ระยะเวลานอนโรงพยาบาล มัธยฐาน 13.0 วัน (Q1-Q3= 7.0-19.0) คุณลักษณะผู้ป่วยที่สัมพันธ์กับ ระยะเวลานอนโรงพยาบาล ได้แก่ ประเภทผู้ได้รับบาดเจ็บ (p <0.001) ระดับความรุนแรงของการบาดเจ็บ (p=0.007) การผ่าตัด (p<0.001) การทำกายภาพบำบัด (p<0.001) และ ภาวะแทรกซ้อน (p<0.001) ส่วนโอกาสรอดชีวิตไม่มีความสัมพันธ์กับระยะเวลานอนโรงพยาบาล ข้อเสนอแนะในการพัฒนาประสิทธิภาพการดูแลผู้ป่วยควรมุ่งที่การป้องกันภาวะแทรกซ้อน จัดบริการฟื้นฟูผู้ป่วยโดยโปรแกรมกายภาพบำบัดที่ีบ้าน และบริการกายภาพบำบัดในชุมชน

The objectives of this retrospective cohort study were to analyze the hospital length of stay (HLOS) and to examine the patients' characteristics and service factors associated with HLOS among road traffic injured patients in Phramongkutklao hospital. The study was based on the hospital's trauma center database with included traffic injured patients who were treated at the emergency room and admitted as inpatients between April 2019 and September 2019, by using a data collection form. The samples were 139 traffic injured patients who were admitted. The HLOS was counted in days. The data were analyzed using descriptive and the analytic statistics test for the means difference via Mann-Whitney U test and Kruskal Wallis test. The results revealed that of the total 139 patients, 89.2% were male, median ages was 30.0 years old (Q1-Q3=24-48 years). 43.9% were at moderate severity level (ISS 9-15), 98.6% had probability of survival (Ps) during 75-100%. The median HLOS was 13.0 days (Q1-Q3=7.0-19.0 days). The patients' characteristics including; types of vehicle (p<0.001), severity level (p=0.007), having surgery (p<0.001), having rehabilitation (p<0.001) and having complication (p<0.001) were associated with longer HLOS significantly. However, the probability of survival (Ps) was not a factor associated with HLOS. The suggestions of this study for developing patient care should focus on prevent of complications in hospital, provide home based program rehabilitation and community based rehabilitation services.
