ประสบการณ์การใช้ยาสมุนไพรของผู้ป่วยความดันโลหิตสูง

Abstract

การวิจัยนี้เป็นการศึกษาประสบการณ์ แนวคิด การให้ความหมายของอาการป่วย และประสบการณ์การใช้ยาสมุนไพรของผู้ป่วยความดันโลหิตสูง ที่กินยาหอมระย่อม จำนวน 15 ราย ที่มารักษาที่คลินิกอายุรเวทแพทย์แผนไทยประยุกต์ ใช้วิธีวิจัยเชิงคุณภาพ เก็บข้อมูลโดยการสัมภาษณ์เชิงลึกและวิเคราะห์ข้อมูลโดยใช้กรอบแนวคิดทฤษฎีปฏิสัมพันธ์เชิงสัญลักษณ์ ผลการศึกษาประสบการณ์การใช้ยาสมุนไพรของผู้ป่วยความดันโลหิตสูง พบว่าผู้ป่วยตระหนักในความผิดปกติที่เกิดขึ้น ได้แก่ อาการมึนศีรษะ นอนไม่หลับ เหนื่อยอ่อนเพลีย ผู้ป่วยจะตีความอาการป่วย จากประสบการณ์แล้วเชื่อมโยงกับอาการป่วยของตัวเองกับการทำงาน กับพฤติกรรมการกินอาหาร การไม่ออกกำลังกาย และการดื่มแอลกอฮอล์ผู้ป่วยประเมินความรุนแรงจากการเห็นอาการป่วยของคนใกล้ชิด เช่น หลอดเลือดสมองแตก อัมพาต ส่วนการให้ความหมายของความดันโลหิตสูงของผู้ป่วย ว่าเป็นอาการป่วยที่มีอาการไม่แน่นอน อาการป่วยเป็นภาวะที่ต้องพึ่งพา พึ่งพิง และเป็นภาระให้คนอื่น เป็นอาการป่วยที่ผู้ป่วยต้องยอมรับและ แสวงหาวิธีการรักษาแบบอื่นๆเช่น การกินอาหาร การออกกำลังกาย การใช้สมุนไพร การเตือนสติให้กลับมาดูแลสุขภาพของตนเอง เช่น การออกกำลังกาย การสวดมนต์ การปรับอาหาร ส่วนประสบการณ์การใช้สมุนไพร พบว่า ผู้ป่วยมีแรงกระตุ้น จากความเป็นธรรมชาติ และความปลอดภัยของสมุนไพร การรับรู้ว่าสมุนไพรรักษาอาการปวดศีรษะ รักษาอาการอ่อนเพลีย ผู้ป่วยยังมีการทบทวนวิธีการอื่นที่ช่วยป้องกันและลดอาการแทรกซ้อนของความดันโลหิตสูง เช่น การกินอาหาร การออกกำลังกาย ส่วนการกินสมุนไพรเป็นเป้าหมายสำคัญที่ผู้ป่วยตัดสินใจเลือกใช้โดยมีการใช้สมุนไพรอย่างเดียว และใช้สมุนไพรคู่กับยาแผนปัจจุบัน ข้อเสนอแนะ: ควรคัดกรองผู้ป่วยความดันโลหิตสูงในกลุ่มผู้ป่วยที่อายุน้อย และควรมีการศึกษาฤทธิ์และความเป็นพิษของยาสมุนไพรตำรับ

This study studied experiences, attitudes, definition of illness and experiences of herbal medicine use in hypertensive 15 patients who have received medication treatment with YahomRayom at Ayurved Thai Traditional Applied Medicine College. This study is a qualitative study and it was conducted with in-depth interview and data analysis using conceptual frameworkof Symbolic Interaction theory. The behavioral study of these hypertensive patients found that the patients were aware of the abnormalities such as dizziness, insomnia and fatigue. The patients would interpret their symptoms based on their experiences and related them with their work, eating behaviors, lack of exercise and alcohol beverage consumption. The patients evaluated severity of the symptoms based on the results such as hemorrhagic strokes and paralysis that they have witnessed on their acquaintances. As for the definition of hypertension, the patients defined it as an illness with uncertain symptoms. The patients also thought of the illness as requiring them to be dependent and a burden to other people. Hypertension required them to accept and seek alternative treatments such as nutrition, exercise, herbal usage and an appreciation in taking care of their health by doing exercise, praying and diet adjustment. For experiences in herbal medicine use, it found that the patients were motivated by nature, safety, acknowledgement of the benefits in terms of headaches and tiredness relief form herbal medicine. The patients also reviewed of other methods known to prevent and reduce complications of hypertension such as nutrition and exercise. However, the use of herbal medicine became the major choice for the patients, either for standalone use or with modern medicines. Suggestion: There should be a screening for hypertension in younger patients. In addition, studies of pharmacological reactions and toxicity of herbal recipes should also be conducted.
