ปัจจัยทำนายความสามารถในการปฏิบัติกิจวัตรประจำวันของผู้สูงอายุหลังผ่าตัดสะโพก

Abstract

วัตถุประสงค์ของการศึกษานี้เพื่อศึกษาอำนาจทำนายของความปวดหลังผ่าตัด คุณภาพการนอนหลับ และภาวะโภชนาการต่อความสามารถในการปฏิบัติกิจวัตรประจำวันในผู้ป่วยสูงอายุหลังผ่าตัดสะโพก โดยใช้แนวคิดของทฤษฎีการเปลี่ยนผ่าน ประชากรในการศึกษานี้เป็นผู้สูงอายุที่ได้รับการผ่าตัดรักษากระดูกสะโพกหักในโรงพยาบาลระดับตติยภูมิ 3 แห่ง กลุ่มตัวอย่างมี 77 คน เครื่องมือที่ใช้ในการศึกษาครั้งนี้ประกอบด้วย แบบประเมินความปวด คุณภาพการนอนหลับ ภาวะโภชนาการ และความสามารถในการปฏิบัติกิจวัตรประจำวัน วิเคราะห์ข้อมูลโดยใช้สถิติถดถอยพหุคูณแบบขั้นตอนเดียว (Enter Regression)ผลการวิจัยพบว่ากลุ่มตัวอย่าง ร้อยละ 66.2 มีความปวดเล็กน้อย ร้อยละ 72.7 มีคุณภาพการนอนหลับไม่ดี ร้อยละ 49.4 เสี่ยงต่อภาวะขาดสารอาหาร ร้อยละ 66.2 มีการพึ่งพาผู้อื่นเล็กน้อย ความปวดหลังผ่าตัด คุณภาพการนอนหลับ ภาวะโภชนาการ สามารถร่วมทำนายความสามารถในการปฏิบัติกิจวัตรประจำวันในผู้ป่วยสูงอายุหลังผ่าตัดสะโพกได้ร้อยละ 24 (R 2 =0.24, F=7.47, p<0.01)การนำผลของวิทยานิพนธ์ไปใช้ด้านการปฏิบัติการพยาบาล พยาบาลควรส่งเสริมการประเมินคุณภาพการนอนหลับ ภาวะโภชนาการ และการจัดการด้านความปวดหลังผ่าตัด เพื่อให้เกิดการฟื้นฟูสภาพด้านการปฏิบัติกิจวัตรประจำวันในผู้สูงอายุหลังผ่าตัดสะโพก ด้านการวิจัย ควรศึกษาวิจัยเชิงทดลองโดยสร้างโปรแกรมเกี่ยวกับการส่งเสริมอาหารที่มีประโยชน์ ร่วมกับโปรแกรมการจัดการเกี่ยวกับการนอนไม่หลับ และการจัดการด้านความปวดหลังผ่าตัด เพื่อส่งเสริมการเปลี่ยนผ่านในผู้ป่วยสูงอายุหลังผ่าตัดสะโพก

The objective of this study was to predict the effects of postoperative pain, quality of sleep and nutritional status support on the activities of daily living in older adults after hip surgery using the concept of Transition Theory. The population in this study was older adults who had undergone surgical treatment of hip fractures in three tertiary hospitals. The purposive samples were a total of 77 older adults. The instruments used in this study consisted of VAS, PSQI, MNA-SF and MBAI. The data were analyzed using Enter Regression. The results of this study revealed that mild postoperative pain was 66.2%.Meanwhile, 72.7% of the sample experienced poor quality of sleep. 49.4% were at risk for malnutrition and 66.2% were slightly dependent on other persons. Postoperative pain, quality of sleep and nutritional status were able to co-predict the activities of daily living of older adult patients after hip surgery at 24% (R2 =.24, F = 7.47, p< .01). IMPLICATION OF THE THESIS. Implications for nursing practice are that nurses should assess and promote quality of sleep and nutritional status and postoperative pain for older adults with hip fracture after surgery for the rehabilitation of activities of daily living in older adult after hip surgery. Furthermore, there should be the development of experimental research to create nutritional management program and insomnia and postoperative pain management programs to promote healthy transition in older adults after hip surgery.
