ชาติพันธุ์กับปัญหาการรับบริการฝากครรภ์ของมารดาชาวพม่า : กรณีศึกษาชุมชนแห่งหนึ่งในจังหวัดกาญจนบุรี

Abstract

ปัญหาการฝากครรภ์ไม่ครบตามเกณฑ์ของแม่ยังเป็นปัญหาสำคัญในประเทศกำลังพัฒนาหลาย ประเทศโดยเฉพาะในกลุ่มที่มีเศรษฐานะยากจนซึ่งรวมถึงปัญหาแม่ที่เป็นกลุ่มชาติพันธุ์และเป็นคนย้ายถิ่นใน บริเวณชายแดนไทย-พม่าของประเทศไทย การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อศึกษาปัญหาการฝากครรภ์ไม่ครบตามเกณฑ์ของแม่ชาวพม่าและ ปัจจัยกำหนดคือความเป็นชาติพันธุ์ชาวพม่าและบริบทชีวิตของการเป็นคนย้ายถิ่นโดยใช้วิธีการวิจัยเชิงคุณภาพ เก็บข้อมูลด้วยการสัมภาษณ์เชิงลึกและการสังเกตแบบไม่มีส่วนร่วมกับแม่ชาวพม่าที่ฝากครรภ์ไม่ครบตามเกณฑ์ จำนวน 11 รายผลการศึกษาพบว่าแม่ชาวพม่าทั้ง 11 รายมีการฝากครรภ์ไม่ครบตามเกณฑ์ โดยปัจจัยทีมีอิทธิพลสำคัญต่อปัญหาดังกล่าวคือวัฒนธรรมความเชื่อในเรื่องของการตั้งครรภ์การ ฝากครรภ์และการดูแลทารกปัญหาการเข้าไม่ถึงบริการอันเนื่องมาจากปัญหาการสื่อสารเพราะพูดภาษาไทยไม่ได้ ปัญหาการเดินทางที่ยากลำบากและห่างไกลสถานบริการตลอดจนปัญหาความยากจนที่ทำให้ต้องให้ความสำคัญ กับการหารายได้เลี้ยงชีพมาก่อนการหยุดงานเพื่อมาฝากครรภ์การวิจัยนี้มีข้อเสนอแนะว่าระบบบริการสาธารณสุข โดยเฉพาะการฝากครรภ์ควรมีการปรับเกณฑ์การประเมินและรูปแบบการให้บริการที่สอดคล้องกับสภาพ วัฒนธรรมสังคมเศรษฐกิจและการเดินทางของแม่ชาวพม่าที่เป็นคนย้ายถิ่นให้มากขึ้นโดยเน้นการให้บริการเชิงรุก บูรณาการการทำงานให้มีความสอดคล้องกันระหว่างหน่วยงานในพื้นที่ปรับปรุงระบบการสื่อสารประชาสัมพันธ์ ให้มีความเหมาะสมโดยใช้ภาษาพม่าเป็นต้น

An incomplete schedule of antenatal care service utilisation among mothers living with povertyis still a major problem in many developing countries,especially the ethnic migrants living alongtheThailand-Myanmar border. This study aimed to understand problems of incomplete scheduling of antenatal care service use among Burmese mothers, and the influencing factors which include the mother's ethnic backgrounds and migration contexts. A qualitative approach using in-depth interviews and non-participant observation as data collection techniques was employed eleven mothers with incomplete schedules of antenatal care service use were selected as informants. Results of the study revealed the different patterns of incompleteness of antenatal care service use by all mothers. Major factors accounting for the problems include the mother's cultural beliefs concerning pregnancy, antenatal care, and baby care. Inaccessibility to antenatal care service is one of the main factors were found, being caused by language barriers and communication problems, long distancs, lack of transportation, and poverty. Those factors simultaneously caused the mothers to give priority to work instead of coming to the health centre for antenatal care service. Recommendations of this study include the improvement of the existing antenatal care service schedule,which should be more responsive to the socio-cultural and economic contexts of the migrant mothers, i.e., by providing more out-reach service and using the local language in the health education message.
