Physician and nurse assistant scheduling problem in hospital

Abstract

Nowadays, the number of patients visiting Thai hospitals has constantly increased, consequently all government hospitals are confronted with workload and resignation problems. To cope up with high turnover rate among physicians, many recruiting and scheduling systems are being implemented. However, physician scheduling is time-consuming, and usually cannot satisfy overall physicians' requirements. Hospital needs a new scheduling system to address this concern. The new scheduling system should be convenient, quick, and suitable for overall physicians' requirements, and must align with the current workflow. This research focused on physician scheduling in Outpatient Department in a government hospital, and aimed to create a scheduling system that maximizes overall physicians' satisfaction by matching timeslots and skills of nurse assistant with each physician requirement. The study employed Analytic Hierarchy Process, Integer Linear Programming and Patient Forecasting in order to obtain the optimal scheduling system. The result indicates that the group of skills and competencies of nurse assistants are medical skills and basic capability. In addition, after new scheduling system implementation, the overall physicians' satisfaction increases to 16.87 percent. The future research should study about an increase in work efficiency regarding matching skills and competencies between physicians and nurse assistants.

ปัจจุบันจำนวนผู้ป่วยที่เข้ารับการรักษาในสถานพยาบาลไทยมีแนวโน้มเพิ่มมากขึ้นอย่างต่อเนื่อง ส่งผลให้แพทย์ในโรงพยาบาลรัฐมีภาระงานเพิ่มขึ้นทุกปี ซึ่งการลาออกของแพทย์จากภาระงานที่เพิ่มขึ้นทาให้โรงพยาบาลรัฐต้องจัดหาแพทย์เข้ามาทดแทน ควบคู่ไปกับการจัดตารางการทำงานของแพทย์ให้มีประสิทธิภาพมากขึ้น อย่างไรก็ตามการจัดตารางการทำงานของแพทย์เป็นงานที่ใช้ระยะเวลานาน และไม่สามารถตอบสนองต่อความต้องการของแพทย์โดยรวมได้ โรงพยาบาลจึงต้องการระบบการจัดตารางที่มีความสะดวกรวดเร็ว และสามารถตอบสนองต่อความต้องการของแพทย์โดยรวม โดยไม่กระทบกับระบบการทำงานที่มีอยู่ในปัจจุบัน งานวิจัยนี้มุ่งศึกษาเกี่ยวกับการจัดตารางการทำงานของแพทย์ในแผนกผู้ป่วยนอกของโรงพยาบาลรัฐ และมีเป้าหมายเพื่อสร้างระบบการจัดตารางการทำงานที่ก่อให้เกิดความพึงพอใจสูงสุดให้แก่แพทย์ โดยมีการพิจารณาถึงตารางเวลาและความสอดคล้องของสามารถในการทำงานของผู้ช่วยพยาบาลกับความต้องการของแพทย์ เครื่องมือที่ใช้ในงานวิจัยนี้ คือ การตัดสินใจโดยใช้กระบวนการลำดับขั้นเชิงวิเคราะห์ (Analytic Hierarchy Process) โปรแกรมเชิงเส้น (Integer Linear Programming) และการพยากรณ์จำนวนของคนไข้ (Patient Forecasting) ผลศึกษาพบว่า ความเชี่ยวชาญทางด้านการแพทย์ (Medical skills) และ ทักษะทางด้านพื้นฐาน (Basic Capability) เป็นปัจจัยที่มีผลต่อการเลือกผู้ช่วยในการทำงาน ภายหลังการใช้ระบบจัดตารางการทำงาน พบว่า ความพึงพอใจของแพทย์โดยรวมมีอัตราเพิ่มขึ้นจากเดิมร้อยละ 16.87 งานวิจัยในอนาคตควรที่จะศึกษาเกี่ยวกับการเพิ่มประสิทธิภาพการทำงานของการจัดตารางการทำงานร่วมกันระหว่างแพทย์และผู้ช่วยพยาบาล
