Abstract
This research aimed 1) to study the development of health service system quality in the present cardiovascular health area, 2) to study supporting factors and analyze problems or obstacles of development the quality of health service system, and 3) to propose a model for improving the quality of the seamless health service system for multidisciplinary in health area. The model development consisted of 4 processes, namely, study of relevant data and documents, in-depth interviews, group discussions, and professional networking teams. This research led to a new service system design to assess the appropriateness of the model used by experts or practitioners in the public health profession. The data were analyzed both quantitatively and qualitatively. The results showed that the new model of health service system development was the 3 C Acts: 3 I Support model, including Commitment Approach, Collaborative Care for the Care System, Care System, Integration, Information to improve, Innovation and Continuing Care for best results including efficiency, quality and security to target. The new model was brought to the trial area, and the conclusion of new health service system development showed improvement of care for patients with cardiovascular disease. It is suggested to develop more coaching system, use of information program, Rehabilitation Services after surgery and a cardiovascular disease preparation course in the professional field in order to increase the capacity of the network to be more effective.
การวิจัยนี้เป็นการวิจัยและพัฒนา โดยมีวัตถุประสงค์ 1.เพื่อศึกษาการพัฒนาคุณภาพระบบบริการสุขภาพ ในเขตสุขภาพ ประเด็นโรคหัวใจและหลอดเลือดในปัจจุบัน 2. เพื่อศึกษาปัจจัยสนับสนุน วิเคราะห์ปัญหา/ อุปสรรคของการพัฒนาคุณภาพระบบบริการสุขภาพ 3. เพื่อเสนอแนะรูปแบบการพัฒนาคุณภาพระบบบริการสุขภาพแบบไร้รอยต่อของเครือข่ายวิชาชีพในเขตสุขภาพประเด็นโรคหัวใจและหลอดเลือด มี 4 กระบวนการ คือ ศึกษาข้อมูลและเอกสารที่เกี่ยวข้อง, สัมภาษณ์เชิงลึก, ประชุมสนทนากลุ่มทีมเครือข่ายวิชาชีพ และออกแบบระบบบริการใหม่ และ ประเมินความเหมาะสมในการนำไปใช้ วิเคราะห์ข้อมูลทั้งเชิงปริมาณและเชิงคุณภาพ ผลการศึกษา พบว่ารูปแบบการพัฒนาคุณภาพระบบบริการสุขภาพแบบใหม่ คือ 3 C Act : 3 I Support model ประกอบด้วยการสร้างข้อตกลง (Commitment Approach), ทำงานร่วมกัน (Collaborative) ในการจัดระบบการดูแลกลุ่มเป้าหมาย (Care System) โดยมีการบูรณาการเชื่อมต่อการทำงานร่วมกันทั้งแนวดิ่งและแนวราบ (Integration) ใช้ข้อมูลปัญหาในพื้นที่มาวิเคราะห์ (Information) เพื่อพัฒนาปรับปรุง สร้างนวัตกรรม (Innovation) การดูแลสุขภาพอย่างต่อเนื่องเพื่อให้เกิดผลลัพธ์ที่ดีที่สุด ทั้งประสิทธิภาพ,คุณภาพ และความปลอดภัยแก่กลุ่มเป้าหมาย และควรพัฒนาเพิ่มเติมเรื่องระบบพี่เลี้ยง การใช้โปรแกรมข้อมูลสารสนเทศ ระบบบริการฟื้นฟูสมรรถภาพหลังการรักษา หลังการผ่าตัด และจัดทำหลักสูตรเฉพาะโรคหัวใจและหลอดเลือด ในสาขาวิชาชีพต่างๆ เพื่อเพิ่มศักยภาพของเครือข่ายให้มีประสิทธิภาพมากยิ่งขึ้น
