Abstract
The long waiting time of patients in receiving services in the outpatient
department is one of the vital problems in the public hospital in Thailand. This leads to patient dissatisfaction, and some patients leave without getting any services. Their performance directly effects patient satisfaction levels and the reduction of the hospital's income. For these reasons, Thailand's hospitals have been making efforts to improve their service systems. In this thesis, we have studied the patient flows and service processes of the Otolaryngology and Ophthalmology Outpatient Departments in Ramathibodi Hospital in Bangkok. Both departments face a long patient waiting time problem. The objective of this research was to propose new systems in order to reduce the waiting time of patients in both departments. The ARENA simulation software (Version 10.0) was used to study the impacts of new systems on each department. It was found that the new system could be successfully applied to reduce waiting time. In addition, the results in this thesis can be applied to other hospitals to improve their service systems.
ปัญหาหนึ่งที่สำคัญของโรงพยาบาลรัฐบาลในประเทศไทยคือ การรอคอยรับการรักษา ของผู้ป่วยที่นาน ทำให้ผู้ป่วยไม่พอใจและทำให้ผู้ป่วยบางคนเดินออกจากโรงพยาบาลโดยไม่รอรับบริการ พฤติกรรมเหล่านี้ส่งผลกระทบโดยตรงกับระดับความพึงพอใจของผู้ป่วยและการลดลงของรายได้ของโรงพยาบาล ด้วยเหตุผลนี้โรงพยาบาลของไทยได้พยายามปรับปรุงระบบการให้บริการ งานวิจัยนี้ศึกษาการไหลของผู้ป่วยและระบบการให้บริการของหน่วยตรวจผู้ป่วยนอก หู คอ จมูก และตา โรงพยาบาลรามาธิบดี กรุงเทพมหานคร ซึ่งปัจจุบันหน่วยตรวจดังกล่าวประสบปัญหาผู้ป่วยใช้เวลารอคอยในระบบนาน งานวิจัยนี้มีจุดประสงค์เพื่อเสนอระบบใหม่เพื่อลดเวลารอคอยของผู้ป่วยในระบบ ซึ่งทางคณะผู้วิจัยใช้โปรแกรมการจำลองสถานการณ์ ARENA (เวอร์ชัน 10.0) ศึกษาผลกระทบของระบบที่นำเสนอสำหรับหน่วยตรวจทั้งสองหน่วย ผลการศึกษาพบว่าระบบใหม่ สามารถไปประยุกต์ใช้เพื่อลดเวลารอคอยได้เป็นอย่างดี ผลจากงานวิจัยนี้ยังสามารถนำไปประยุกต์ใช้กับโรงพยาบาลอื่นได้อีกด้วย
