Abstract
Cardiac catheterization is commonly used to diagnose the degree of Coronary arteries obstruction. The cardiac catheterization through the wrist artery must be conducted by a highly competent and knowledgeable clinician. Practicing cardiac catheterization with a simulation model improves the experience and learning in a safe setting where the patient is not harmed. The objective of this research is the material development of the simulator for Transradial Catheterization by analyzing three types of silicone rubber. In this study, the test is divided into 12groups depending on the use of puncture and catheter. And evaluated satisfaction with the simulator's effectiveness and utility by 18 physicians from 1st year and 2nd year of the fellowship of Internal medicine Cardiology and the Fellowship Training in Interventional Cardiology participated in testing the simulation model at the Faculty of Medicine Siriraj Hospital. The results showed that the three silicone rubbers were significantly different (P<0.1), the silicone rubber RTV-01 was suitable for creating the skin, and silicone rubber RTV-03 was suitable for creating the vascular. For the satisfaction assessment results of 18 participants by using the Likert scale 5 points, it was found that the satisfaction with the efficacy of the model had an average score of 4.41, and the satisfaction with the model's usefulness had an average score of 4.59. Therefore, the development of cardiac catheterization through the wrist for physicians increases the efficiency of learning in an appropriate environment and enhances the practical experience of the procedure.
การสวนหัวใจเป็นส่วนหนึ่งของการวินิจฉัยเพื่อหาตำแหน่งการอุดตันของหลอดเลือดแดงที่เลี้ยงกล้ามเนื้อหัวใจ โดยการใส่สายสวนหัวใจผ่านทางหลอดเลือดแดงที่ข้อมือจะต้องทำโดยแพทย์ที่มีประสบการณ์และความชำนาญสูง การฝึกหัตถการสวนหัวใจกับหุ่นจำลอง จะช่วยเสริมประสบการณ์ และการเรียนรู้ในสภาพแวดล้อมที่ปลอดภัย ฝึกซ้ำได้โดยไม่ส่งผลอันตรายต่อผู้ป่วย วัตถุประสงค์ของงานวิจัยครั้งนี้คือ การพัฒนาวัสดุที่ใช้ในการสร้างหุ่นจำลองสำหรับการฝึกทักษะการสวนหัวใจผ่านหลอดเลือดแดงเรเดียล โดยวิเคราะห์วัสดุยางซิลิโคน 3 ชนิด แบ่งการทดสอบเป็น 12 กลุ่มโดยใช้การแทงเข็มและใส่สายสวน วัดประสิทธิภาพความพึงพอใจของหุ่นจำลอง ที่คณะแพทยศาสตร์ศิริราชพยาบาล โดยแพทย์ประจำบ้านต่อยอดสาขาอายุรศาสตร์โรคหัวใจ ชั้นปีที่ 1, 2 และแพทย์ประจำบ้านต่อยอดอนุสาขาหัตถการปฏิบัติรักษาโรคหัวใจและหลอดเลือด 18 ท่าน ผลการทดลองพบว่ายางซิลิโคนทั้ง 3 มีค่าแตกต่างกันอย่างมีนัยสำคัญ (P<0.1) คือ RTV-01 เหมาะสำหรับสร้างผิวหนัง และยางซิลิโคน RTV-03 เหมาะสำหรับสร้างหลอดเลือด สำหรับผลการประเมินความพึงพอใจ โดยผู้เข้าร่วมวิจัย 18 ท่าน พบว่าค่าเฉลี่ยความพึงพอใจในประสิทธิภาพของหุ่นจำลอง ร้อยละ 4.41 และค่าเฉลี่ยความพึงพอใจในประโยชน์ของหุ่นจำลอง ร้อยละ 4.59 แปลว่าพึงพอใจมากที่สุด ดังนั้นการพัฒนาหุ่นจำลองในการฝึกหัตถการสำหรับการสวนหัวใจผ่านทางข้อมือแก่แพทย์จึงเพิ่มประสิทธิภาพการเรียนในสภาพแวดล้อมที่เหมาะสม และช่วยเพิ่มประสบการณ์ในการทำหัตถการได้จริง
