Abstract

Malignant spinal cord compression (MSCC) คือ ภาวะการกดเบียดระบบประสาทไขสันหลังซึ่งเกิดจากมะเร็งลุกล้ำกระดูกสันหลังหรือเนื้อเยื่อข้างเคียง ส่งผลให้ผู้ป่วยเกิดทุพพลภาพทั้งในด้านการเคลื่อนไหว การรับรู้ความรู้สึก การขับถ่ายปัสสาวะหรืออุจจาระ หากการดำเนินโรครวดเร็วหรือไม่ได้รับการรักษาที่ทันเวลาอาจก่อให้เกิดภาวะแทรกซ้อน นำไปสู่คุณภาพชีวิตและอัตราการรอดชีวิตที่ลดลง จึงเป็นภาวะฉุกเฉินทางโรคมะเร็งที่มีความสำคัญเร่งด่วน ผู้ป่วยจำเป็นต้องได้รับการวินิจฉัยที่รวดเร็วและให้การรักษาที่เหมาะสมโดยแพทย์สหสาขาวิชาชีพ ได้แก่ อายุรแพทย์โรคมะเร็ง แพทย์รังสีรักษา ประสาทศัลยแพทย์หรือศัลยแพทย์กระดูกสันหลัง แพทย์เวชศาสตร์ฟื้นฟู และควรได้รับการดูแลทางจิตใจร่วมด้วยเพื่อผลการรักษาที่ดีที่สุด

Malignant spinal cord compression (MSCC) is defined as spinal cord compression associated with vertebral metastasis or neoplastic invading surrounding tissue. MSCC may result in several neurological dysfunctions, particularly motor weakness, loss of sensation, bowel and bladder dysfunction. Rapid disease progression or delay in appropriate therapy are associated with high morbidity and mortality. MSCC is an oncologic emergency that requires prompt diagnosis and management. Multidisciplinary approach including medical and radiation oncologists, neurological or orthopedic surgeons, physical rehabilitation and psychosocial providers is required for the best treatment outcomes.
