ประสิทธิภาพของนวัตกรรมใบคัดกรองเพื่อคัดกรองผู้ป่วย Stroke, Sepsis และ STEMI ในผู้ป่วยหน่วยเวชศาสตร์ครอบครัว ศูนย์การแพทย์กาญจนาภิเษก

Abstract

งานวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาความแม่นยำและประสิทธิภาพของแบบประเมิน Early Warning Signs ‘3S’ ซึ่งพัฒนาขึ้นเพื่อช่วยให้บุคลากรสามารถคัดกรองผู้ป่วยกลุ่มโรคเร่งด่วนได้อย่างรวดเร็วและถูกต้อง เพื่อนำข้อมูลไปประกอบการ Triage และจัดลำดับคิวเข้าพบแพทย์ตามมาตรฐาน 30 นาที การศึกษานี้เป็นการศึกษาวิจัยแบบย้อนหลัง (Retrospective Study)โดยแบ่งการเก็บข้อมูลเป็น 3 ระยะ คือระยะพัฒนาใบคัดกรอง ทำการวิเคราะห์ข้อมูลย้อนหลังจากเวชระเบียนผู้ป่วยจำนวน 52 ฉบับที่เข้ารับการรักษาในหน่วยเวชศาสตร์ครอบครัว ศูนย์การแพทย์กาญจนาภิเษก เพื่อนำมาพัฒนาแบบประเมิน Early Warning Signs ‘3S’ ระยะทดสอบความถูกต้องของแบบประเมิน Early Warning Signs ‘3S’ และระยะนำไปใช้กับผู้ป่วยจริงในหน่วยเวชศาสตร์ครอบครัวโดยกลุ่มตัวอย่างประกอบด้วยผู้ป่วยจำนวน 50 ราย แบ่งเป็น 25 รายในกลุ่มที่ได้รับการคัดกรองแบบเดิม และ 25 รายในกลุ่มที่ได้รับการคัดกรองด้วยแบบประเมิน Early Warning Signs ‘3S’ โดยวิธีการ Randomization ผลการวิจัยพบว่า แบบประเมิน Early Warning Signs ‘3S’ มีความแม่นยำในการคัดกรองตรงกับผลการวินิจฉัยของแพทย์ในระดับร้อยละ 100 ในขณะที่การคัดกรองแบบเดิมมีความแม่นยำเพียงร้อยละ 60 และยังพบว่าแบบประเมิน Early Warning Signs ‘3S’ สามารถลดระยะเวลาในการรอคอยพบแพทย์ได้อย่างมีนัยสำคัญ โดยช่วยให้เจ้าหน้าที่นำข้อมูลไปประกอบการ Triage และจัดลำดับคิวกลุ่ม Urgency ให้เข้าพบแพทย์ภายในระยะเวลามาตรฐาน 30 นาที ได้ร้อยละ 100 ซึ่งเป็นระยะเวลาที่เหมาะสมส่งผลต่อความปลอดภัยในชีวิตของผู้ป่วยกลุ่มเร่งด่วน นอกจากนี้ยังมีการเก็บข้อมูลความพึงพอใจของเจ้าหน้าที่ต่อการใช้งานแบบประเมิน Early Warning Signs ‘3S’ พบว่าบุคลากรส่วนใหญ่มีความพึงพอใจในระดับสูง โดยเฉพาะในด้านความสะดวกในการใช้งาน ความครบถ้วนของข้อมูลเกณฑ์ประเมิน และความชัดเจนของแบบฟอร์ม จากผลการวิจัยสามารถสรุปได้ว่า แบบประเมิน Early Warning Signs ‘3S’ เป็นนวัตกรรมที่มีประสิทธิภาพสูงในการคัดกรองผู้ป่วยโรคฉุกเฉินกลุ่ม Stroke, Sepsis และ STEMI โดยสามารถเพิ่มความแม่นยำ ลดระยะเวลารอคอยพบแพทย์ และเป็นเครื่องมือที่ได้รับการยอมรับจากบุคลากรในภาคปฏิบัติ เหมาะสมที่จะนำไปประยุกต์ใช้ในระดับโรงพยาบาลที่มากขึ้น เช่น จุดคัดกรองของหน่วยงานอื่น เผยแพร่ให้เป็นที่แพร่หลาย เพื่อใช้ในระดับจังหวัด ระดับประเทศต่อไป เพื่อเพิ่มคุณภาพของการดูแลผู้ป่วยฉุกเฉินอย่างมีประสิทธิภาพต่อไป

This research aimed to evaluate the accuracy and efficiency of the 3S screening form, which was developed to help personnel screen urgent patients quickly and accurately. The study was designed as Retrospective research, with data collection divided into 3 phases: a retrospective study of the medical records of the development phase of the screening form, the retrospective research data from 52 patients treated at the Family Medicine Unit, Golden Jubilee Medical Center, which can be used to develop an Early Warning Signs ‘3S’ assessment form, the validity testing phase of the Early Warning Signs ‘3S’ assessment form, and the phase the conventional method and 25 patients screened using the 3S screening form, sampling by randomization. The results showed that the 3S screening form achieved 100% accuracy in matching the doctor’s duagnosis, compares to 60% accuracy with the conventional screening method. Furthermore, the 3S screening form significantly reduced the time required for patients to see a doctor. Patients screened with the 3S screening form were able to see a doctor within 30 minutes, which is an appropriate timeframe for urgent patients. Additionally, data on personnel satisfaction with the 3S screening form were collected. Most staff reported high levels of satisfaction, particulary with the form’s ease of use, comprehensive assessment criteria, and clarity. From the research results, it can be concluded that the 3S screening form is a highly effective innovation for screening emergency patients in the Stroke, Sepsis and STEMI groups. It improves diagnosis accuracy, reduce patient wait times, and is a tool that is accepted by personnel in the practical sector. Therefore, It is well-suited for implementation in primary care units to enhance the quality of emergency care.
