Abstract
HIV disclosure, as part of the discussion before having sex, can be an effective approach for HIV prevention as it can lead to more effective condom use and HIV prevention. However, revealing one's HIV status is not easy, especially among men who have sex with men (MSM) and transgender women. In Indonesia, MSM and transgender women have a high vulnerability to stigma and discrimination due to social, cultural, and religious prejudices regarding homosexual behavior. On the other hand, HIV status disclosures have not been explored adequately in the context of Indonesia. This study aimed to examine the factors associated with HIV status disclosure among MSM and transgender women in Bali, Indonesia. This study uses the SEKSI 2019 dataset, which is a cross-sectional online survey among the key population in Bali. The sample that was eligible for this analysis was men who have sex with men during the last six months and men who have sex with non-permanent partners. The Chi-square test was applied to find the association of HIV status disclosure according to the independent variables. Binary logistic regression was employed to find the determinants of HIV status disclosure. We report p-value for bivariate analysis and adjusted odds ratios (AOR) at the 95% confidence interval (CI) for the associations in multivariate analysis. Only 31.6% out of 452 respondents in this study had disclosed their HIV status to their non-permanent partners. The Chi-square test found that the proportion of HIV status disclosure varied according to gender identity, discrimination experience, types of sex partners, method of finding sex partners, gender identity of sex partner, number of male sex partners, social engagement in MSM or transgender communities, and engage in transactional sex (p-value < 0.05). Based on the binary logistic regression, factors associated with increased likelihood of disclosing one’s HIV status among respondents include having engaged in transactional sex (AOR=2.11; 95%CI=1.16–3.86). Conversely, factors associated with decreased likelihood to disclose one’s HIV status include having experienced discrimination (AOR=0.46; 95%CI=0.29–0.73) and finding sex partners from public venues only (AOR=0.14; 95%CI=0.04–0.54). Implication of Thesis: Based on these findings, the intervention can be focused on encouraging the discussion about HIV, promote benefits of HIV status disclosure among these groups, and reduce discrimination, especially by involving MSM and transgender communities in the HIV prevention program
