ความพึงพอใจของผู้ป่วยเรื้อรังที่รับบริการกายภาพบำบัดเยี่ยมบ้านของศูนย์บริการสาธารณสุข สำนักอนามัย กรุงเทพมหานคร

Abstract

การวิจัยครั้งนี้เป็นการวิจัยเชิงสำรวจแบบภาคตัดขวาง (Cross-Sectional Survey) โดยมีวัตถุประสงค์ เพื่อวัดระดับความพึงพอใจของผู้ป่วยเรื้อรังที่รับบริการกายภาพบำบัดเยี่ยมบ้านของศูนย์บริการสาธารณสุข สำนัก อนามัย กรุงเทพมหานคร อธิบายข้อมูลทั่วไป การรับรู้เกี่ยวกับสุขภาพและการเจ็บป่วย ระบบการบริการกายภาพบำบัด เยี่ยมบ้าน และนักกายภาพบำบัด พร้อมทั้งหาความสัมพันธ์ระหว่างปัจจัยดังกล่าวข้างต้นกับความพึงพอใจ กลุ่ม ตัวอย่างคือ ผู้ป่วยเรื้อรังที่เคยรับบริการกายภาพบำบัดเยี่ยมบ้านของศูนย์บริการสาธารณสุข สำนักอนามัย กรุงเทพมหานคร จำนวน 163 คน รวบรวมข้อมูลโดย ใช้แบบสอบถาม ในช่วงระยะเวลาตั้งแต่ 1 สิงหาคม ถึง 30 กันยายน 2557 ได้แบบสอบถามที่สมบูรณ์และสามารถนำมาวิเคราะห์ได้ทั้งหมด 163 ฉบับ (ร้อยละ 100.0) สถิติที่ใช้ คือ ค่าจำนวน ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน วิเคราะห์หาความสัมพันธ์โดยใช้สถิติ t - Test สถิติ F - Test สัมประสิทธิ์สหสัมพันธ์แบบเพียร์สัน ผลการวิจัยพบว่า ผู้ป่วยเรื้อรังที่รับบริการกายภาพบำบัดเยี่ยมบ้าน ของศูนย์บริการสาธารณสุข สำนัก อนามัย กรุงเทพมหานคร มีความพึงพอใจในภาพรวมของบริการกายภาพบำบัดเยี่ยมบ้านของศูนย์ฯ อยู่ในระดับมาก ( x̄ = 4.41) เมื่อวิเคราะห์ความสัมพันธ์ระหว่างข้อมูลทั่วไป การรับรู้เกี่ยวกับสุขภาพและการเจ็บป่วย ระบบการบริการ กายภาพบำบัดเยี่ยมบ้าน และนักกายภาพบำบัด พบว่า สถานภาพสมรสและรายได้เฉลี่ยของครอบครัวต่อเดือนที่ แตกต่างกันมีความพึงพอใจที่แตกต่างกันอย่างมีนัยสำคัญทางสถิติ (p-value<0.05) ด้านการรับรู้เกี่ยวกับสุขภาพและ การเจ็บป่วยของตนเอง ด้านนักกายภาพบำบัดมีความสัมพันธ์เชิงบวกกับความพึงพอใจโดยรวมในระดับปานกลาง อย่างมีนัยสำคัญทางสถิติ (r = 0.654, p-value < 0.001 และ r = 0.616, p-value < 0.001 ตามลำดับ) ขณะที่ด้านระบบ บริการกายภาพบำบัดเยี่ยมบ้านมีความสัมพันธ์เชิงบวกกับความพึงพอใจโดยรวมในระดับสูงอย่างมีนัยสำคัญทางสถิติ (r = 0.717, p-value < 0.001) ข้อเสนอแนะจากการวิจัยควรพัฒนาประสิทธิภาพในการประชาสัมพันธ์โดยมีป้ายแสดงกิจกรรมการ ดำเนินงาน แจกเอกสารแผ่นพับ และชี้แจงว่ามีบริการอะไรบ้างทั้งในคลินิกศูนย์บริการสาธารณสุขและในชุมชน พัฒนาศักยภาพของนักกายภาพบำบัดโดยส่งเสริมการจัดประชุมวิชาการ สร้างวัฒนธรรมการบริการให้กับเจ้าหน้าที่ที่ ให้บริการ สร้างแรงจูงใจในการปฏิบัติงานให้กับเจ้าหน้าที่ พัฒนาระบบการให้บริการและการประสานงานระหว่าง ทีมสหวิชาชีพโดยนำระบบนัดหมายมาใช้ให้เกิดประโยชน์อย่างเป็นรูปธรรม

The study was cross-sectional survey research with the objective of measuring the level of patient's satisfaction with physiotherapy home health care for chronically ill patients provided by the Public Health Center, Department of Health, Bangkok Metropolis. This describes general information and perceptions about health/illness, the physiotherapy home health care system and the physiotherapist; including studying the association between these factors with patient's satisfaction. The studied samples were 163 chronically ill patients who had the experience of receiving physiotherapy home health care from the Public Health Center, Department of Health, Bangkok Metropolis. Questionnaires were used as the data collecting tool during 1st August to 30th September 2557 B.E. There were 163 completed and returned questionnaires (100%). Statistics used for data analysis were percentage, mean, standard deviation, t-test, F-test and Pearson's Correlation Coefficient. The study found that the overall patient's satisfaction was at a high level ( x̄ = 4.41), analysis of the association between general information, perceptions about health/illness, the physiotherapy home health care system and the physiotherapist with patient's satisfaction found that the patients with different marital status and average family income had a different level of satisfaction with the service with a statistical significance. Self-perception of health/illness and opinion about physiotherapists had a medially positive association with satisfaction with a statistical significance (r = 0.654, p-value < 0.001 and r = 0.717, p-value < 0.001 consecutively). Research recommendations are; there should be an improvement of the efficiency of public relations relating to the program e.g. board displaying activities, distribution of pamphlet explaining about services available in the Public Health Center and in the community. Increase the potentiality of the physiotherapists through academic conferences, create a service culture among service providers, create work motivation for personnel, improve the service system and inter-disciplinary coordination through a concrete service appointment system.
