Abstract
The prevalence and financial burden of non-communicable diseases (NCDs) are increasing worldwide. While evidence from many studies indicated that households with NCD members are at greater financial risk, little information is available in Vietnam. Therefore, this study was conducted to examine the magnitude of financial burden due to out-of-pocket (OOP) expenditure among households with NCD patients and its related factors in Vietnam by using secondary data which was obtained from a cross-sectional household survey in 6 provinces in 2015. The selection criteria was households having at least one member suffering from hypertension or diabetes. The final sample size of 1791 households and 2016 patients was analyzed using STATA software with descriptive and multiple regression analysis. In this study, 61.6% of patients were elderly. Hypertensive patients accounted for 87.7%, and the health insurance coverage among NCD patients was 85.0%. There was 85.6% households paid OOP for health care, with an average expense of 488 USD per year. With the threshold of 40% of capacity to pay, 8.7% and 3.6% of households with NCD members facing catastrophic and impoverishing health expenditure due to OOP payments, respectively. The size of household, living area, wealth quintiles, number of NCD members, sex and age of household head, and use of private and inpatient services were significantly associated with adverse economic outcomes. IMPLICATION OF THESIS: The policy should better targeted towards households with NCDs who are living in rural and remote, mountainous areas, having more NCD members and belong to low quintile groups. National programs and policies related to financial support through health insurance and basic benefit packages at primary health care to increase the health service accessibility of NCD patients and their households should be considered. There is a need to implement further research with more updated data and collection of patterns of the costs of health services for better support this population.
