ผลของโปรแกรมส่งเสริมการเลี้ยงลูกด้วยนมแม่โดยบิดามีส่วนร่วมต่ออัตราการเลี้ยงลูกด้วยนมแม่อย่างเดียว 1 เดือน และการรับรู้การสนับสนุนจากบิดาในมารดาที่มีบุตรคนแรก

Abstract

การศึกษาครั้งนี้ มีวัตถุประสงค์เพื่อศึกษาผลของโปรแกรมส่งเสริมการเลี้ยงลูกด้วยนมแม่โดยบิดามีส่วนร่วมต่ออัตราการเลี้ยงลูกด้วยนมแม่อย่างเดียว 1 เดือน และการรับรู้การสนับสนุนจากบิดา ในมารดาที่มีบุตรคนแรก รูปแบบการศึกษาเป็นการวิจัยกึ่งทดลอง ใช้แบบจำลองการส่งเสริมสุขภาพเป็นกรอบแนวคิดการวิจัย กลุ่มตัวอย่างเป็นมารดาหลังคลอดที่พักฟื้นในหอผู้ป่วยหลังคลอด โรงพยาบาลพระจอมเกล้า จังหวัดเพชรบุรี จำนวน 54 ราย แบ่งเป็นกลุ่มควบคุม และกลุ่มทดลอง กลุ่มละ 27 ราย กลุ่มควบคุมได้รับการพยาบาลตามปกติ กลุ่มทดลองได้รับโปรแกรมส่งเสริมการเลี้ยงลูกด้วยนมแม่โดยบิดามีส่วนร่วมร่วมกับการพยาบาลตามปกติ เก็บ รวบรวมข้อมูลโดยใช้แบบสอบถามข้อมูลส่วนบุคคล แบบประเมินประสิทธิภาพการดูดนมของทารก แบบสัมภาษณ์ปัญหาในการเลี้ยงลูกด้วยนมแม่ แบบสัมภาษณ์การให้อาหารทารก และแบบสอบถามการรับรู้การสนับสนุนจากบิดา เปรียบเทียบสัดส่วนการเลี้ยงลูกด้วยนมแม่อย่างเดียว 1 เดือน และคะแนนเฉลี่ยการรับรู้การสนับสนุนจากบิดา ระหว่างกลุ่มควบคุมและกลุ่มทดลอง โดยใช้การทดสอบค่าซี (Z-test) และการทดสอบค่าที (independent t-test) ผลการศึกษาพบว่า มารดากลุ่มทดลองมีสัดส่วนการเลี้ยงลูกด้วยนมแม่อย่างเดียว 1 เดือนมากกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ (Z=8.01; p<.001) และมีคะแนนเฉลี่ยการรับรู้การสนับสนุนจากบิดาสูงกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ (t=11.397; p<.001) จากการวิจัยครั้งนี้ มีข้อเสนอแนะว่า โปรแกรมส่งเสริมการเลี้ยงลูกด้วยนมแม่โดยบิดามีส่วนร่วมสามารถเพิ่มอัตราการเลี้ยงลูกด้วยนมแม่อย่างเดียว 1 เดือน และการรับรู้การสนับสนุนจากบิดาได้ จึงควรส่งเสริมให้พยาบาลนำโปรแกรมนี้ ไปใช้ในปฏิบัติการพยาบาล เพื่อช่วยให้มารดาที่มีบุตรคนแรกประสบความสำเร็จในการเลี้ยงลูกด้วยนมแม่อย่างเดียวต่อไป

The main objectives of this study were to evaluate the effectiveness of breastfeeding promotion program with father involvement on the rate of exclusive breastfeeding at one month postpartum and perceptions of father’s support among the first-time mothers. This study was a quasi-experimental design. Health Promotion Model (HPM) provided the theoretical framework of this study. The study sample included 54 first-time mothers admitted to the postpartum unit at Phrachomkhao hospital in Phetchaburi and divided into control and experimental groups, with 27 participants in each group. The sample in the control group received only routine nursing care, whereas those in the experimental group received breastfeeding promotion program with father involvement combined with routine nursing care. Data were collected by demographic characteristic questionnaire, effective suckling checklist, problem with breastfeeding interview form, feeding interview form and perception of father’s support questionnaire. Z-test and independent t-test were used to analyze the data. Results showed that the proportion of exclusive breastfeeding at one month postpartum in the experimental group was significantly higher than those in the control group (Z=8.01; p<.001). Average scores of perception of father’s support were significantly higher in the experimental group than those in the control group (t=11.397; p<.001). The results of this study showed that breastfeeding promotion program with father involvement can increase the rate of exclusive breastfeeding at one month postpartum and perception of father’s support. Therefore, nurses should apply this program to nursing practice to help first-time mothers succeed in continuing to breastfeeding.
