ประสิทธิผลโรงพยาบาลส่งเสริมสุขภาพตำบลในงานป้องกันและควบคุมวัณโรค เขตสุขภาพที่ 6

Abstract

การวิจัยเชิงอรรถาธิบายแบบภาคตัดขวางนี้ มีวัตถุประสงค์เพื่อประเมินประสิทธิผลโรงพยาบาลส่งเสริมสุขภาพตำบล (รพสต.) ในงานป้องกันและควบคุมวัณโรค เขตสุขภาพที่ 6 และหาความสัมพันธ์กับแรงสนับสนุนทางสังคมและความสามารถของตนเอง กลุ่มตัวอย่างคือผู้รับผิดชอบงานป้องกันและควบคุมวัณโรค แห่งละ 1 คน สุ่มตัวอย่างแบบชั้นภูมิ รวบรวมข้อมูลโดยใช้แบบสอบถามแบบตอบด้วยตนเอง ส่งทางไปรษณีย์ ได้รับตอบกลับจำนวน 328 ชุด (ร้อยละ 85.86) วิเคราะห์ข้อมูลด้วยสถิติสัมประสิทธิ์สหสัมพันธ์ของเพียร์สันและการถดถอยพหุ ผลการวิจัยพบว่า ประสิทธิผล รพสต. ในงานป้องกันและควบคุมวัณโรค อยู่ในระดับสูง (M=3.51±0.39) แรงสนับสนุนทางสังคมอยู่ในระดับปานกลาง (M=3.40±0.51) และ ความสามารถของตนเองอยู่ในระดับสูง (M=3.57±0.46) แรงสนับสนุนทางสังคม และความสามารถของตนเองมีความสัมพันธ์ทางบวกระดับต่ำ กับประสิทธิผล รพสต. ในงานป้องกันและควบคุมวัณโรค (r=0.358, r= 0.407, p-value < 0.05) ทั้งสองตัวแปรนี้สามารถร่วมกันอธิบายความผันแปรของประสิทธิผลโรงพยาบาลส่งเสริมสุขภาพตำบล ในงานป้องกันและควบคุมวัณโรค ได้ร้อยละ 21.7 (Adjusted R2 = 0.217, p-value < 0.001) การวิจัยครั้งนี้มีข้อเสนอแนะ คือ ผู้บริหารควรกำหนดนโยบายพัฒนาความสามารถของผู้ปฏิบัติงานด้านการควบคุมวัณโรคและสนับสนุนให้ได้รับข้อมูลวิธีการปฏิบัติงานที่ดีจากหน่วยงานอื่น ๆ เพื่อเพิ่มประสิทธิผลงานป้องกันและควบคุมวัณโรค

This cross-sectional explanatory study aims to asses effectiveness of Tambon (subdistrict) health promotion hospitals (THPH) in tuberculosis prevention and control (TPC), in health service provider board 6 and examine the association between social support and competence of the officers. A sample of 328 (response rate 85.86%) officers who had responsibility in tuberculosis prevention and control was selected by a stratified random sampling method. Data were collected using a self-administered questionnaire and analyzed. Using Pearson product-moment correlation and multiple liner regression to identify associated factors and predictors. The results revealed that the effectiveness of THPH in tuberculosis prevention and control was at a high level (M=3.51±0.39): the social support was at moderate level ( M=3.40±0.51) and competence of the officers was at a high level (M=3.57±0.46). The social support and competence of the officers were the positive significant factors associated with competence of the officers (r = 0.358, r = 0.407, p-value < 0.05), and they were the predictors that could explain the 21.7% variance in the effectiveness of THPH (Adjusted R2 = 0.217, p-value < 0.001). In summary, we recommended to consider a policy on improvement focusing on competence of officers and support the data improving in TPC area.
