ผลของโปรแกรมส่งเสริมพฤติกรรมการดูแลเท้าและแผลที่เท้าด้วยตนเองของผู้ป่วยเบาหวาน โรงพยาบาลกระทุ่มแบน จังหวัดสมุทรสาคร

Abstract

ภาวะแทรกซ้อนที่สำาคัญของโรคเบาหวานคือ แผลเรื้อรังที่เท้า การวิจัยกึ่งทดลองแบบสองกลุ่มวัด ก่อนและหลังการทดลอง เพื่อศึกษาผลของโปรแกรม ส่งเสริมพฤติกรรมการดูแลเท้าและแผลที่เท้าด้วย ตนเองของผู้ป่วยเบาหวาน ที่ประยุกต์ทฤษีการรับรู้ ความสามารถของตนเองศึกษาในผู้ป่วยเบาหวาน จำานวน 32 ราย ที่มีแผลเท้าในระดับ 3 ณ คลินิก เท้าเบาหวาน โรงพยาบาลกระทุ่มแบน แบ่งเป็นกลุ่ม ทดลอง 17 ราย กลุ่มเปรียบเทียบ 15 ราย ใช้เวลา การศึกษา 8 สัปดาห์ ประกอบด้วยกิจกรรมกลุ่ม 4 ครั้ง และโทรศัพท์ติดตาม 4 ครั้ง เก็บรวบรวมข้อมูล ก่อน-หลังการทดลองด้วยแบบสัมภาษณ์ วิเคราะห์ข้อมูล ด้วยสถิติ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน Chi-square Test, Independent T-test, Paired Sample T-test, Wilcoxon Signed Ranks Test, Mann-Whitney U Test, และ Z-test ภายหลัง การทดลอง พบว่า กลุ่มทดลองมีคะแนนเฉลี่ยการรับรู้ ความสามารถของตนเอง ความคาดหวังในผลของ การปฏิบัติ และการปฏิบัติพฤติกรรมการดูแลเท้าและ แผลที่เท้าด้วยตนเองสูงกว่ากลุ่มเปรียบเทียบอย่างมี นัยสำาคัญทางสถิติ (p<0.05) ผู้ป่วยทั้งสองกลุ่มไม่พบ การเกิดแผลใหม่ และกลุ่มทดลองไม่พบการลุกลาม ของแผล ในขณะที่กลุ่มเปรียบเทียบร้อยละ 13.3 พบการลุกลามของแผล แต่ไม่แตกต่างกันทางสถิติ (p = 0.12) ดังนั้นโปรแกรมจึงสามารถส่งเสริมพฤติกรรม การดูแลเท้าและแผลที่เท้าด้วยตนเองของผู้ป่วย เบาหวานได้

A major complication of diabetes is chronic foot ulcer. This quasi-experimental, two group pretest-posttest research design was conducted to study the effect of a promoting program for self-care behaviors and foot ulcer among patients with diabetes applying self-efficacy theory. The target groups comprised 32 diabetic patients with foot ulcer at level 3 at the Diabetes Foot Clinic, Krathumbaen Hospital. The experimental group consisted of 17 patients and 15 patients comprised the comparison group. The 8-week program conducted group activities, followed up by telephone, four times. Data were analyzed using percentage, mean, standard deviation, Chi-square test, Independent t-test, Paired Sample t-test, Wilcoxon signed ranks test, Mann-Whitney U test, and z-test. After the experiment the experimental group had signifi cantly higher mean scores of self-efficacy, outcome expectations and behavior of foot self-care and foot ulcer than the comparison group (p <0.05). The diabetic patients in the two groups did not present new ulcers. The progression of ulcer was not found in the experimental group while it was in the comparison group (13.3%) without signifi cance (p = 0.12). Therefore, the program could promote self-care of foot and foot ulcer of diabetic patients.
