Abstract
This study aims to compare qualitative and quantitative results of 99mTc-MIBI Myocardial Perfusion Imaging Single Photon Emission Computed Tomography/Computed Tomography (MPI SPECT/CT) in the diagnosis of coronary artery disease (CAD) using 3 different reconstruction techniques, including filtered back projection (FBP), iterative reconstruction with attenuation correction (IRAC), and iterative reconstruction non-attenuation correction (IRNC). The retrospective study was performed in 263 patients who underwent MPI SPECT/CT at the Division of Nuclear Medicine, Siriraj Hospital. All MPI images obtained from each reconstruction technique were independently assessed by two experienced nuclear physicians without knowing the patients’ clinical information and cardiac angiography (CAG) results. The differences in image quality, diagnosis, the level of confidence, and quantitative results obtained from the 3 techniques were compared. Statistical significance level is defined as p < 0.05. Good image quality was found in FBP (96.16%) > IRNC (94.56%) > IRAC (91.25%) techniques, while artifacts were detected in IRAC (8.75%) > IRNC (5.44%) > FBP (3.84%) techniques. The interobserver agreement and level of diagnostic confidence were highest in the IRAC technique. The diagnostic performance using FBP, IRNC, and IRAC techniques was shown as follows: sensitivity (80.85%, 68.09%, 55.32%); specificity (65.71%, 65.71%, 72.86%); PPV (61.29%, 57.78%, 57.14%); NPV (83.64%, 75.41%, 70.83%); and accuracy (71.79%, 66.67%, 65.81%). There were overall strong to very strong agreements between quantitative results from all techniques. However, statistically significant differences in the functional results were still detected when using different reconstruction techniques, including percent of defect, LV mass, EDV, ESV, LVEF, and TID. Overall, good agreement between results from the different reconstruction techniques in MPI SPECT/CT study can be expected; however, differences in qualitative and quantitative results can occur. While the IRAC technique improves the specificity and level of diagnostic confidence for CAD by MPI SPECT/CT, it may decrease the sensitivity and accuracy, especially in patients with enhanced artifacts suspected from over-attenuation correction and BMI > 25 kg/m2, leading to misinterpretation. Therefore, IRAC images should be used with caution in interpreting MPI SPECT/CT. Moreover, including FBP images in the routine interpretation process may help optimize the diagnostic performance of CAD.
การศึกษาเพื่อเปรียบเทียบความแตกต่างของผลการตรวจทั้งเชิงคุณภาพและเชิงปริมาณที่ได้จากการสร้างภาพในการวินิจฉัยภาวะกล้ามเนื้อหัวใจขาดเลือดจากการตรวจ Myocardial Perfusion Imaging Single Photon Emission Computed Tomography (MPI SPECT/CT) ด้วยเทคนิคการสร้างภาพที่แตกต่างกัน 3 เทคนิค คือ 1. Filtered back projection (FBP) 2. Iterative reconstruction ที่ใช้ attenuation correction (IRAC) และ 3. Iterative reconstruction ที่ไม่ใช้ attenuation correction (IRNC) เป็นการศึกษาแบบย้อนหลังในผู้ป่วยที่มารับการตรวจ MPI SPECT/CT ที่สาขาวิชาเวชศาสตร์นิวเคลียร์ โรงพยาบาลศิริราช เพื่อวินิจฉัยภาวะกล้ามเนื้อหัวใจขาดเลือดจำนวน 263 ราย ที่ได้รับการถ่ายภาพ MPI SPECT/CT ตามวิธีการมาตรฐาน จากนั้นข้อมูลการตรวจจะได้รับการสร้างภาพด้วยเทคนิคทั้ง 3 เทคนิค ได้แก่ Filtered back projection (FBP) และ Iterative reconstruction แบบที่ใช้และไม่ใช้ CT attenuation correction มาแก้ค่าการลดทอนของรังสี ซึ่งภาพที่ได้จากวิธีการสร้างภาพทั้ง 3 เทคนิคจะได้รับการประเมินคุณภาพของภาพ การวินิจฉัยภาวะกล้ามเนื้อหัวใจขาดเลือด ระดับความมั่นใจในการวินิจฉัย และค่า LV mass, EDV, ESV, LVEF และ TID ด้วยเกณฑ์เดียวกันโดยแพทย์ผู้เชี่ยวชาญทางด้านเวชศาสตร์นิวเคลียร์ 2 ท่าน โดยมีเกณฑ์มาตรฐาน (gold standard) ได้แก่ ผลการตรวจ Coronary Angiography (CAG) และ/หรือ ผลการติดตามอาการและอาการแสดงของภาวะกล้ามเนื้อหัวใจขาดเลือดที่ระยะเวลา 1 ปี ส่วนข้อมูลเชิงปริมาณต่าง ๆ ที่ได้จากการตรวจ MPI จากการสร้างภาพทั้ง 3 เทคนิคจะถูกนำมาวิเคราะห์ผลเปรียบเทียบทางสถิติ โดยกำหนดค่านัยสำคัญทางสถิติที่ระดับ p-value น้อยกว่า 0.05 ผลการประเมินคุณภาพของภาพพบว่าเทคนิค FBP (96.16%) > IRNC (94.56%) > IRAC (91.25%) ในขณะที่พบสิ่งแปลกปลอมในภาพ (artifacts) มากในเทคนิค IRAC (8.75%) > IRNC (5.44%) > FBP (3.84%) และพบว่าเทคนิค IRAC มีระดับความเชื่อมั่นและความสอดคล้องของผู้ประเมินในการวินิจฉัยสูงกว่าเทคนิคอื่น ๆ จากการศึกษานี้ได้แสดงประสิทธิภาพในการวินิจฉัยของเทคนิค FBP, IRNC และ IRAC โดยค่า sensitivity (80.85%, 68.09%, 55.32%); specificity (65.71%, 65.71%, 72.86%); PPV (61.29%, 57.78%, 57.14%); NPV (83.64%, 75.41%, 70.83%) และ accuracy (71.79%, 66.67%, 65.81%) ตามลำดับ นอกจากนี้การประเมินค่าเชิงปริมาณของร้อยละของ defect, LV mass, EDV, ESV, LVEF และ TID จากทั้งสามเทคนิคพบว่ามีความสัมพันธ์ที่แข็งแรงถึงแข็งแรงมาก แต่อย่างไรก็ตามยังคงมีความแตกต่างอย่างมีนัยสำคัญทางสถิติ จากผลการศึกษาสรุปว่าทั้งสามเทคนิคมีความสัมพันธ์ที่ดีแต่สามารถเกิดความแตกต่างของค่าเชิงคุณภาพและเชิงปริมาณได้ ในขณะที่เทคนิค IRAC สามารถช่วยเพิ่มค่า specificity และระดับความมั่นใจของแพทย์ได้ แต่ในทางกลับกันก็ลดค่า sensitivity และ accuracy โดยเฉพาะอย่างยิ่งในผู้ป่วยที่มีการสูงขึ้นของ artifacts เนื่องจากการแก้ค่าการลดทอนทางรังสีที่มากเกินไปและในผู้ป่วยที่มี BMI > 25 kg/m² ซึ่งนำไปสู่การแปลผลที่ผิดพลาด ดังนั้นเทคนิค IRAC ควรได้รับการใช้อย่างระมัดระวังสำหรับ MPI SPECT/CT และควรใช้เทคนิค FBP ควบคู่ในการประเมินผล ซึ่งจะช่วยเพิ่มประสิทธิภาพในการตรวจวินิจฉัยโรคหลอดเลือดหัวใจ (CAD)
