Abstract
Autosomal dominant polycystic kidney disease (ADPKD) is one of the most common life-threatening human genetic disorders, characterized by the progressive development of multiple abnormal fluid-filled cysts in the kidneys. The major cause of ADPKD, accounting for approximately 85% of all cases, is due to mutations of polycystic kidney disease 1 (PKD1) gene. PKDI is mapped to 16pl3.3 with the size of 54 kb, containing 46 exons and transcribing a 14-kb mRNA. It encodes, polycystin-l, an integral membrane protein with possible functions in cell-cell or cell-matrix interaction and in signal transduction involved in normal tubulogenesis. Isolation and analysis of PKDI have encountered a great difficulty since approximately three-fourth at its 5 end is reiterated and at least three highly homologous genes are present at another region on the same chromosome. The attempts to isolate the entire PKDI coding sequence have never previously been successful, delaying the progression in analysis of PKDI mutations. The study in this thesis was initiated by addressing this problem. The fact that information about PKDI mutations in Thai ADPKD patients has not been available inhibits the understanding of its molecular pathogenesis. A method of long reverse transcriptase and polymerase chain reaction (RT-PCR) to selectively amplify the entire PKDI coding sequence from its mRNA transcripts, prepared from peripheral blood Iymphocyte, has successfully been developed in this study. The principle of this method is based on the use of a pair of PCR primers, one primer specific to the sequence in the 3 unique region of PKDI and the other to the sequence in the homologous region at a distance of about 13.6 kb apart. The amplified full-length PKDI cDNA was fractionated into nine overlapping fragments by nested PCRs, appropriate for further analysis. This procedure was applied to analyze RNA samples prepared from 10 unrelated Thai ADPKD patients. The important advantage of this procedure for the mutation analysis was realized by the discoveries of two cases with defective RNA processing, in which the resulting defective RNA processing could be demonstrated ex vivo. The RNA splicing defects and the causative PKDI mutations in these two families were characterized. In addition, an unusual alternative PKDI-RNA splicing observed in many patients was also studied. The results showed that in the first family (PK015) the observed 74 nucleotide (nt) deletion of exon 14 in the PKDI-mRNA transcripts was most likely to occur from a nucleotide substitution (A->T) at a splice acceptor site in IVS 13, IVS 13-2A>T. This splice acceptor site was abolished and a cryptic splice acceptor site present in exon 14 was used, leading to the deletion of PKDI mRNA. This deletion would cause frameshift and truncation of polycystin-1, comprising only 1,074 amino acids. Allele-specific amplification (ASA) was developed for direct detection of this mutation in the PK015 family, which was found to segregate with the disease. In the second family (PK009), the 291-nt deletion in PKDI-mRNA transcripts was found to result from exon 43 skipping caused by deletion of 20-bp in intron 43, IVS43(+5 to +14)del20. This skipping might occur from inadequate length of the deleted intron 43 for spliceosome formation, since the deletion did not disrupt the donor or acceptor splice site or branch site in this intron. As the result of exon 43 skipping, the translated polycystin-1 would have in-frame deletion of 97 amino acids in the transmembrane (TM) and loop regions, confirming the importance of this domain. The intronic deletion in this family could directly be detected by genomic DNA amplification, which was found to inherit with the disease. These two mutations are likely to cause loss of polycystin-1 function, which would support the two-hit model of PKDl pathogenesis. The intensive studies of PKD1 mutations will provide a better understanding ofpolycystin-1 function and molecular pathogenesis of PKD1.
โรคไตเป็นถุงน้ำชนิด autosomal dominant (autosomal dominant polycystic kidney disease, ADPKD) เป็นโรคพันธุกรรมที่คุกคามต่อชีวิตมนุษย์ที่พบได้บ่อยที่สุดโรคหนึ่ง ลักษณะของโรคคือมีถุงน้ำจำนวนมากในไตทั้งสองข้าง ประมาณ 85% ของ ADPKD เกิดจากมิวเตชั่น ของยีน PKD1 ซึ่งเป็นยีนที่อยู่บนตำแหน่งโฆรโมโซมที่ 16p13.3 มีขนาด 54 กิโลเบส, ประกอบด้วย 46 exon และมี mRNA ขนาด 14 กิโลเบส ซึ่งถอดรหัสเป็นโปรตีนของเยื่อหุ้มเซลล์ ชื่อ polycystin-1 ซึ่งอาจจะมีหน้าที่เกี่ยวกับปฏิสัมพันธ์ระหว่าง cell กับ cell หรือ cell กับ matrix และมีบทบาทในการถ่ายทอดสัญญาณที่เกี่ยวข้องกับการสร้างและพัฒนาการของท่อไต ที่ผ่านมาการแยกยีน PKD1 เพื่อศึกษาและวิเคราะห์ทำได้ยาก เนื่องจากประมาณ 3 ใน 4 ของยีนทางด้าน 5 ถูกลอกเลียนโดยยีนอื่นอีกอย่างน้อย 3 ยีนบนโฆรโมโซมเดียวกันซึ่งคล้ายกับ ยีน PKD1 มาก ด้วยเหตุนี้จึงไม่เคยมีผู้ใดสามารถแยก coding sequence ของยีน PKD1 ที่สมบูรณ์ได้สำเร็จมาก่อน การศึกษามิวเตชั่นของยีน PKD1 จึงเป็นไปอย่างล่าช้า การศึกษาในวิทยานิพนธ์นี้เริ่มต้นโดยปรารภปัญหาดังกล่าวและการที่ยังไม่มีข้อมูลเกี่ยวกับ มิวเตชั่นของยีน PKD1 ในผู้ป่วยไทยโรค ADPKD ทำให้ขาดความเข้าใจถึงพยาธิกำเนิดใน ระดับอณูของโรค วิธี long reverse transcription and polymerase chain reaction (long RT-PCR) ได้รับการพัฒนาขึ้นเพื่อเลือกเพิ่มปริมาณ coding sequence ของยีน PKD1 ทั้งหมดจาก mRNA ที่เตรียมจากเซลล์ลิมโฟชัยต์เป็นผลสำเร็จ หลักการของวิธีคือเลือกใช้คู่ของ PCR primer ซึ่งข้างหนึ่งมีความจำเพาะกับลำดับเบสในส่วน 3 ที่มีลักษณะเฉพาะของยีน PKD1 กับอีกข้างหนึ่งที่จำเพาะกับลำดับเบสในส่วนของยีนที่มีความซ้ำซ้อน ซึ่งอยู่ห่างจากกันประมาณ 13.6 กิโลเบส หลังจากนั้นจึงนำ full-length cDNA ของยีน PKD1 ที่เพิ่มปริมาณแล้ว มาแยกส่วนจำนวน 9 ชิ้นซึ่งเหลื่อมกันด้วยวิธี nested PCR เพื่อความเหมาะสมในการศึกษาต่อไป วิธีการนี้ได้นำมาใช้วิเคราะห์ตัวอย่างอาร์เอ็นเอที่ได้จากผู้ป่วยไทยโรค ADPKD จำนวน 10 คน ซึ่งไม่ได้เป็นญาติกัน ประโยชน์ที่สำคัญของวิธีการนี้ในการศึกษามิวเตชั่นเป็นที่ตระหนัก จากการค้นพบความผิดปกติในกระบวนการตัดต่ออาร์เอ็นเอที่สามารถนำมาศึกษาได้ภายนอกร่างกาย ในผู้ป่วย 2 ราย ความผิดปกติในการตัดต่ออาร์เอ็นเอและมิวเตชั่นที่เป็นสาเหตุของโรคใน 2 ครอบครัวนี้ได้ถูกวิเคราะห์ในรายละเอียด นอกจากนี้ยังได้วิเคราะห์สาเหตุของการตัดต่ออาร์เอ็นเอ ที่แตกต่างจากปกติซึ่งพบในผู้ป่วยหลายรายด้วย ผลการวิเคราะห์ปรากฎว่าในครอบครัวแรก (PK015) ซึ่งมีการขาดหายของลำดับนิวคลีโอไทด์จำนวน 74 นิวคลีโอไทด์ของ exon 14 ใน PKD1 transcripts พบว่ามีสาเหตุจาก nucleotide substitution (A>T) ที่ splice acceptor site ของ intron 13 (IVS13-2A>T) มิวเตชั่นนี้ทำให้ splice acceptor site ไม่ถูกใช้งาน และมีการใช้ cryptic splice acceptor site ใน exon 14 แทน จึงทำให้มีการขาดหายของ ลำดับนิวคลีโอไทด์ใน mRNA ผลของมิวเตชั่นนี้ทำให้เกิด frameshift และโปรตีน polycystin-1 สั้นลงเหลือกรดอะมิโนเพียง 1,074 ตัว การตรวจหามิวเตชั่นนี้โดยตรงในครอบครัว PK015 ด้วย วิธี allele-specific amplification (ASA) ที่ได้พัฒนาขึ้น พบว่ามีการถ่ายทอดของมิวเตชั่น ไปกับโรค ส่วนในครอบครัวที่สอง (PK009) ซึ่งมีการขาดหายของลำดับนิวคลีโอไทด์จำนวน 291 นิวคลีโอไทด์ใน mRNA นั้น พบว่าเกิดจากการข้ามเว้น (skipping) ของ exon 43 ที่มีสาเหตุ จากการขาดหายของลำดับนิวคลีโอไทด์จำนวน 20 คู่เบสใน intron 43 [IVS43(+5 to +14) del20] การข้ามเว้น exon 43 นี้น่าจะเกิดจากขนาดของ intron ที่สั้นเกินไปสำหรับการก่อตัว ของ spliceosome เนื่องจากตำแหน่งของ donor และ acceptor splice site หรือ branch site ใน intron ไม่ได้ถูกทำลายจากการขาดหายของ intron ผลของการข้ามเว้น exon 43 ทำให้โปรตีน polycystin-1 ที่ได้มีกรดอะมิโนหายไปแบบ in-frame จำนวน 97 ตัวในบริเวณ ของ transmembrane และ loop ซึ่งเป็นการยืนยันความสำคัญของ domain นี้ต่อหน้าที่ของโปรตีน การขาดหายของลำดับนิวคลีโอไทด์ใน intron ในครอบครัวนี้สามารถตรวจได้โดยตรงด้วยวิธีเพิ่ม ปริมาณ genomic DNA ซึ่งพบว่ามีการถ่ายทอดไปกับโรค โดยสรุปมิวเตชั่นทั้งสองชนิดที่พบในการ ศึกษานี้น่าจะทำให้ polycystin-1 สูญเสียความสามารถในการทำหน้าที่ (loss-of-function) ซึ่งสนับสนุน two-hit model ในพยาธิกำเนิดของโรค PKD1 การศึกษามิวเตชั่นของยีน PKD1 อย่างจริงจังจะช่วยทำให้เข้าใจหน้าที่ของ polycystin-1 และกลไกของพยาธิกำเนิดระดับอณูของ โรค PKD1 ชัดเจนขึ้น
