Abstract
The aim of this study was to develop an automatic calculation of T2* relaxation time acquired from emulated iron-overload phantoms (T2* phantoms) for quality control (QC) of body iron measurement. The study was divided into developing part for optimization of automated parameters and testing part for evaluation of the automation. The images used in this study consisted of real and simulated images, which were acquired in compliance and noncompliance with acquisition protocol of the previous semi-automatic method. The calculation comprised of three processes, phantom background separation, phantom boxes separation and phantom boxes identification. The first process aimed to separate backgrounds from foregrounds using automatic Otsu's thresholding method. In this process, the first echo-time image offered the best thresholding result. All foregrounds could be extracted but some backgrounds still remained and caused some fused regions. Next process, hence, was performed to separate these regions by using erosion and dilation operations. The phantom boxes in all tested conditions could be correctly separated when thresholded images were eroded to 33% and dilated back to 64% of box size using the circle structural element. In the last process, all phantom boxes could be successfully identified by sorting of T2* values using 8-connectivity operation. Furthermore, for noncompliant resolutions and qualities, automatic calculation could be performed effectively even on pixel size or SNR of images were as high as 2.5 times or as lower as 15% of the compliant image, respectively. The analysis time of automatic method was about 116 seconds per series that was less than around 300 seconds in the previous one. The automatic method, hence, could reduce the restriction of the previous method, especially on image acquisition part as expected, and could be reliably performed with faster calculation time. The automatic method, thus, is more suitable for routine QC task and can be fully implemented for routine usage as desired.
การศึกษานี้มีวัตถุประสงค์เพื่อพัฒนาวิธีคำนวณค่า T2* relaxation time แบบอัตโนมัติจากสารจำลองภาวะเหล็กเกิน (T2* phantom) ในงานควบคุมคุณภาพการตรวจวัดปริมาณธาตุเหล็กในร่างกาย การศึกษานี้ถูกแบ่งออกเป็นส่วนของการพัฒนาสำหรับการกำหนดค่าตัวแปรที่เหมาะสมของวิธีแบบอัตโนมัติ และส่วนของ การทดสอบสำหรับประเมินประสิทธิภาพของวิธีแบบอัตโนมัติ ภาพที่ถูกใช้ในการศึกษานี้ประกอบด้วยทั้งภาพจริงและภาพที่จำลองขึ้น ซึ่งถูกถ่ายขึ้นทั้งที่ตรงและไม่ตรงตามข้อกำหนดของวิธีแบบกึ่งอัตโนมัติ การคำนวณนี้ประกอบด้วย 3 ขั้นตอน คือ phantom background separation, phantom boxes separation และ phantom boxes identification ขั้นตอนแรกมีวัตถุประสงค์เพื่อแยกส่วนของพื้นหลังออกจากข้อมูลภาพโดยใช้ automatic Otsu's thresholding method ในขั้นตอนนี้ภาพที่ echo time แรกให้ผลที่ดีที่สุด โดยข้อมูลภาพถูกแยกออกมาทั้งหมด แต่พื้นหลังบางส่วนยังเหลืออยู่และทำให้เกิดบริเวณที่เชื่อมติดกันระหว่างกล่อง ดังนั้นขั้นตอนต่อมาจึงถูกเสนอขึ้นเพื่อแยกบริเวณดังกล่าวโดยใช้การ erosion และ dilation กล่อง phantom ในทุกเงื่อนไขที่ถูกนำมาทดสอบจะถูกแยกออกจากกันได้เมื่อภาพที่ถูก threshold ถูก erode ไปที่ร้อยละ 33 และถูก dilate กลับมาที่ร้อยละ 64 ของพื้นที่กล่อง โดยใช้ structural element รูปวงกลม สำหรับขั้นตอนสุดท้าย กล่องทุกกล่องสามารถถูกระบุตัวตนได้อย่างถูกต้องด้วยการเรียงลำดับค่า T2* โดยใช้หลักการ 8-connectivity นอกจากนี้ วิธีคำนวณนี้ยังสามารถวิเคราะห์ภาพที่มีความละเอียดและคุณภาพไม่ตรงตามข้อกำหนดได้แม้จะมี pixel size หรือ SNR ใหญ่กว่า 2.5 เท่าหรือต่ำกว่าร้อยละ 15 ของค่าที่กำหนด ตามลำดับเวลาที่ใช้ในการวิเคราะห์ภาพของวิธีแบบอัตโนมัติเท่ากับประมาณ 116 วินาทีต่อ series ซึ่งน้อยกว่าวิธีเดิมที่เท่ากับประมาณ 300 วินาทีต่อ series ด้วยเหตุนี้ วิธีแบบอัตโนมัติจึงสามารถลด ข้อจำกัดของวิธีเดิมได้โดยเฉพาะในขั้นตอนของการถ่ายภาพ และสามารถวิเคราะห์ได้อย่างน่าเชื่อถือในเวลาที่น้อยกว่าวิธีแบบอัตโนมัติจึงเหมาะสมกับงานควบคุมคุณภาพมากกว่า และสามารถนำไปใช้งานได้จริงตามที่คาดหวัง
