Abstract
This retrospective study was to assess diagnostic performance of three different techniques of parathyroid scintigraphy, including dual-tracer subtraction (DT), 99mTc-MIBI single-tracer dual-phase (DP), and 99mTc-MIBI SPECT/CT using pathological diagnosis as a gold standard. Out of 630 consecutive patients who underwent parathyroid scintigraphy, 166 patients who were older than 18 years old, and had parathyroid surgery within eight months were reviewed. All the parathyroid scintigraphic images were blindly reinterpreted by two nuclear medicine physicians. A total of 382 parathyroid specimens were analyzed, consisting of 320 hyperplastic glands, 51 parathyroid adenomas, two parathyroid carcinomas, and nine normal parathyroid tissues. According to the lesion-based analysis, the overall sensitivities for DT, DP, and SPECT/CT techniques were 53.35%, 45.04%, and 64.08%, respectively. Nevertheless, subgroup analysis for parathyroid adenoma showed higher percentage sensitivities of 90.20, 84.31, and 98.04 respectively while those for hyperplastic glands were 47.19, 38.44, and 58.44, respectively. In summary, the overall sensitivities of 99mTc-MIBI SPECT/CT were the highest followed by DT, and then DP techniques. The sensitivities were clearly higher in the adenoma group than in the hyperplastic group.
การศึกษานี้เป็นการศึกษาวิจัยย้อนหลัง เพื่อประเมินประสิทธิภาพการวินิจฉัยของการตรวจพาราไทรอยด์สแกนทัRง 3 เทคนิค (เทคนิค dual-tracer subtraction (DT) เทคนิค single-tracer dual-phase (DP) และเทคนิค SPECT/CT) โดยใช้ผลการตรวจทางพยาธิวิทยาที่ได้จากการผ่าตัดเป็นตัววัดมาตรฐาน จากการศึกษาในผู้ป่วยทั้งหมด 630 ราย ที่ได้รับการตรวจพาราไทรอยด์สแกน มีผู้ป่วย 166 ราย ที่อายุมากกว่า 18 ปี และมีผลทางพยาธิวิทยาของต่อมพาราไทรอยด์ภายในระยะเวลา 8 เดือนหลังการตรวจพาราไทรอยด์สแกน โดยแพทย์ทางเวชศาสตร์นิวเคลียร์ 2 ท่าน ซึ่งไม่ทราบข้อมูลทางคลินิก จะทำการอ่านภาพพาราไทรอยด์สแกนใหม่ โดยชิ้นเนื้อ 382 ชิ้นที่ได้รับการวิเคราะห์ แบ่งออกเป็น hyperplasia 320 รอยโรค, adenoma 51 รอยโรค carcinoma 2 รอยโรค และ พาราไทรอยด์ที่ไม่มีพยาธิสภาพ 9 ชิ้น ผลจากการวิเคราะห์แต่ละรอยโรค พบความไวของเทคนิค DT DP และ SPECT/CT ร้อยละ 53.35, 45.04 และ 64.08 ตามลำดับ และเมื่อทำการวิเคราะห์แบบแบ่งกลุ่มพบว่าความไวในกลุ่ม adenoma ร้อยละ 90.20, 84.31 และ 98.04 ส่วนความไวในกลุ่มของ hyperplasia ร้อยละ 47.19, 38.44 และ 58.44 ตามลำดับ โดยสรุปการตรวจพาราไทรอยด์สแกนด้วยเทคนิค SPECT/CT จึงให้ความไวสูงสุด รองลงมา คือ DT และ DP ตามลำดับ และพบว่าความไวในกลุ่ม adenoma สูงกว่าในกลุ่ม hyperplasia อย่างชัดเจน
