    Are educational games more effective than traditional teaching for promoting learning in medical and dental undergraduates?

    Objective: This evidence-based review aims to explore whether educational games are effective in medical and dental undergraduates. Materials and methods: This review conducted a narrative synthesis using the ESRC method. Medline, Embase, and SCOPUS databases were searched using inclusion and exclusion criteria during the period of 1st January 1980 - 28th February 2013. The quality of papers was assessed using the critical appraisal tool by CASP and 2010 CONSORT guidelines. The strength of evidences was assessed using EPPI and GRADE approaches. Results: A total of 391 citations were founded from the search strategy. However, after the processes of screening and eligibility assessment, two randomised controlled trials were included for this review. The first study was conducted in medical undergraduates while the second one was performed in dental undergraduates. Both studies found no significant difference in knowledge improvement after the immediate post-test. Retention of knowledge was investigated only in the first study (medical undergraduates), and it was found that the game group performed better. Only the second study (dental undergraduates) also explored the practical skill and found that there was no significant difference between two groups. Regarding the satisfaction measurement, students had positive attitudes towards the use of educational games in both studies. Conclusion: Although education games are not significantly different from traditional education in the knowledge improvement, there should be an application of educational games in medical and dental education, as educational games can motivate and engage students. However, further studies are required to evaluate the effectiveness of the use of educational games in medical and dental undergraduates.

    Prevalence of Vitamin D insufficiency and low bone mineral density in elderly Thai nursing home residents

    The development of community care model of sub-district health promoting hospitals for older persons

    Thai government is improving public health service systems for better quality and efficiency by upgrading health centers to sub-district health promoting hospitals (SHPH). Healthcare service for older persons must be holistic service combining services for consistency with the needs and problems faced by older persons. This study aimed to investigate community care models of SHPH for older persons in order to manage and provide sustainable care for older persons by using limited resources with quality. The study was using design as mixed method. Quantitative data were obtained via questionnaire from 104 healthcare providers for older persons of each SHPH who had worked more than one year and were selected by stratified sampling from all thirteen districts of Kanchanaburi Province.The qualitative data were collected from four focus groups of purposive key informants. Data analysis used descriptive statistics and content analysis. The model was examined by a consensus of persons involved in the local public hearing. The results revealed the model called the community care model“SHPH for Older Persons” consist of collaborative work by four organizations from the government sector with agencies responsible for providing care for older persons. Every sector must set six strategies and ten factors of success to support SHPH operations in order to provide holistic care for every older persons. The importance of care, family members and communities must be willing to volunteer and participate in providing community care for older persons. The results suggest that The government should set policies and support SHPH, local administrative organizations including related agencies to build community strength in providing care for older persons by developing the knowledge of community leaders, enhancing volunteer capacity, enable linkages in the work of every sector to arrange strong elderly service systems in communities.

    BACKGROUND: Clinical case treatment of malaria infections where Plasmodium falciparum and Plasmodium vivax are sympatric has achieved effective reductions in P. falciparum prevalence and incidence rates, but has been less successful for P. vivax. The high transmissibility of P. vivax and its capacity to relapse have been suggested to make it a harder parasite species to control. METHODS: A clinical malaria case treatment programme was carried out over a decade in a Karen community composed of seven hamlets on the Thai-Myanmar border. RESULTS: From 1994 to 2004, prevalence rates of both P. falciparum and P. vivax decreased by 70-90% in six of the seven study hamlets, but were unchanged in one hamlet. Overall, incidence rates decreased by 72% and 76% for P. falciparum and P. vivax respectively over the period 1999-2004. The age-incidence and prevalence curves suggested that P. vivax was more transmissible than P. falciparum despite a greater overall burden of infection with P. falciparum. Male gender was associated with increased risk of clinical presentation with either parasite species. Children (< 15 years old) had an increased risk of presenting with P. vivax but not P. falciparum. CONCLUSION: There was a considerable reduction in incidence rates of both P. vivax and P. falciparum over a decade following implementation of a case treatment programme. The concern that intervention methods would inadvertently favour one species over another, or even lead to an increase in one parasite species, does not appear to be fulfilled in this case.
