Purpose: To investigate the relationships between age, physiological deterioration, co-morbidity and emergency room discharge destination among patients with trauma. Design: Descriptive correlation design. Methods: The sample composed of 300 patients with traumatic injuries in emergency department, Bach Mai Hospital in Hanoi, Vietnam. Data were collected from the patient’s hospital chart. Spearman’s Rho was employed to test the relationships among studies variables. Main findings: Approximately 60% of patients with traumatic injuries (59.7%) were admitted in hospital, while 35.4% received surgery and/or admitted to intensive care unit. About 40.3% received treatment and were discharged from emergency department to home. Physiological deterioration as measured by Modified Early Warning Score, age, and co-morbidity were positively correlated with emergency room discharge destination (rs = .38, rs = .14, rs = .16, p < .05). Conclusion and recommendations: Modified Early Warning Score should be used to classify trauma injuries patients on their arrival at the emergency department. Hence, the patients can receive appropriate treatment at the right time. Co-morbid diseases should be routinely assessed in all trauma injuries patients on arrival at the emergency room.

Background: The ST-segment elevation myocardial infarction (STEMI) fast track care recommend door to needle within 30 minutes. The current guideline is implemented in the community hospitals for timely reperfusion therapies, however, evaluation on its quality is limited. Objective: To evaluate quality of care for patients with STEMI in community hospitals, Thailand. Method: A descriptive design was conducted in three community hospitals, where distance from primary percutaneous coronary intervention (PCI)-capable hospital are more than 120 minutes. Consecutive sampling was used to select sample of patients with STEMI from October 1, 2016, to Mach 31, 2017. Data were collected from STEMI patients in 3 community hospitals using questionnaire and medical record reviews. Results: Thirty-two STEMI patients participated in this study. The mean age of the patients was 62.5 ± 11.4 years. Most of them (87.5%) used self-transportation. Approximately 51.7% received streptokinase within time, while the average door to needle time was 48.4 minutes. The mean total ischemic time was 246.2 ± 365.7 minutes, and 18.8% of patients died in-hospital. Conclusions: This study showed that door to needle time, door to balloon time, door to ECG, door to referral time, and total ischemic time were longer than recommended time. The main reasons were patients delay and system delay particular delayed diagnosis.

(2015) Vanida Prasert; Farsai Chanjaruporn; วนิดา ประเสริฐ; ฟ้าใส จันท์จารุภรณ์; Mahidol University. Faculty of Pharmacy. Department of Pharmacy
Tambon Health Promoting Hospitals (THPHs) are the first level of public health service system. Before the health service system reform, the THPHs confronted inefficient drug management. Presently, drug management in THPHs develops and expands responsibility to conform health system reformation in Thailand. This study aimed to systematically review the drug management in THPHs to know the current situation of drug management including the efficiency of drug management and patient satisfaction after the reform of the health service system. A literature search was conducted in the ThaiLIS and the Health Systems Research Institute databases during 2004 to 2015. Results revealed the sixteen articles which met the inclusion criteria. The six dimensions of drug management tasks in THPHs which were used as the criteria consist of drug system management, medical supply management, inventory management, pharmaceutical care, home care pharmacy services and rational drug use promotion. It was found that most THPHs performed efficiently for drug management thus patients satisfied for their services. Moreover, some THPHs performed inefficiently on medical supply and inventory management. The problems and obstacles also revealed inadequacy of the staff and budget. Therefore, government should focus on proper resource allocation and provide crucial supports to enhance efficiency and quality of treatment to the patients

Are educational games more effective than traditional teaching for promoting learning in medical and dental undergraduates?
(2014-09) Kawin Sipiyaruk; Khan, Ehsan Ullah; Kawin Sipiyaruk; Mahidol University. Faculty of Dentistry
Objective: This evidence-based review aims to explore whether educational games are effective in medical and dental undergraduates. Materials and methods: This review conducted a narrative synthesis using the ESRC method. Medline, Embase, and SCOPUS databases were searched using inclusion and exclusion criteria during the period of 1st January 1980 - 28th February 2013. The quality of papers was assessed using the critical appraisal tool by CASP and 2010 CONSORT guidelines. The strength of evidences was assessed using EPPI and GRADE approaches. Results: A total of 391 citations were founded from the search strategy. However, after the processes of screening and eligibility assessment, two randomised controlled trials were included for this review. The first study was conducted in medical undergraduates while the second one was performed in dental undergraduates. Both studies found no significant difference in knowledge improvement after the immediate post-test. Retention of knowledge was investigated only in the first study (medical undergraduates), and it was found that the game group performed better. Only the second study (dental undergraduates) also explored the practical skill and found that there was no significant difference between two groups. Regarding the satisfaction measurement, students had positive attitudes towards the use of educational games in both studies. Conclusion: Although education games are not significantly different from traditional education in the knowledge improvement, there should be an application of educational games in medical and dental education, as educational games can motivate and engage students. However, further studies are required to evaluate the effectiveness of the use of educational games in medical and dental undergraduates.
