    Validation and factor structure of the Thai version of the EURO-D scale for depression among older psychiatric patients

    Objectives: To assess the concurrent and the construct validity of the Euro-D in older Thai persons. Method: Eight local psychiatrists used the major depressive episode section of the Mini International Neuropsychiatric Interview to interview 150 consecutive psychiatric clinic attendees. A trained interviewer administered the Euro-D. We used receiver operating characteristic (ROC) analysis to assess the overall discriminability of the Euro-D scale and principal components factor analysis to assess its construct validity. Results: The area under the ROC curve for the Euro-D with respect to major depressive episode was 0.78 [95% confidence interval (CI) 0.70-0.90] indicating moderately good discriminability. At a cut-point of 5/6 the sensitivity for major depressive episodes is 84.3%, specificity 58.6%, and kappa 0.37 (95% CI 0.22-0.52) indicating fair concordance. However, at the 3/4 cut-point recommended from European studies there is high sensitivity (94%) but poor specificity (34%). The principal components analysis suggested four factors. The first two factors conformed to affective suffering (depression, suicidality and tearfulness) and motivation (interest, concentration and enjoyment). Sleep and appetite constituted a separate factor, whereas pessimism loaded on its own factor. Conclusion: Among Thai psychiatric clinic attendees Euro-D is moderately valid for major depression. A much higher cut-point may be required than that which is usually advocated. The Thai version also shares two common factors as reported from most of previous studies.

    Relationship between child rearing and child nutritional status during the first year of life in Thailand

    This descriptive study was conducted to determine relationship between child rearing and child nutritional status during the first year of life. A total of 4,245 cohort children were collected between July 2000 and June 2002 based on the Prospective Cohort of Thai Children. 60 twin infants and 35 dead and abnormal children were excluded. Then, the remaining 4,150 children were conducted for data analysis. The statistics was used by Chi-square tests and Multiple Logistic Regressions were used for identifying influential predictor and child nutritional status (underweight, stunting and wasting) at the first year of age. The finding showed that 8.3% of the children were underweight (weight for age), 9.5% of the children were stunting (height for age) and 5.7% of the children were wasting (weight for height) according to WHO reference. After adjusting the potential confounding factors in the multiple logistic regression, this study found that birth weight was the most significant risk factor related to all three child nutritional status such as underweight (Adj. OR= 10.07, 95% CI= 2.87-35.28), stunting (Adj. OR= 4.49, 95% CI= 1.16-17.39) and wasting (Adj. OR= 3.94, 95% CI= 1.24-12.49). In addition, the significant factor associated with child underweight in the final model was controlling of sleeping time by using rational style (Adj. OR=4.71, 95% CI=1.16-19.10), and for wasting status was types of main caregivers (relative) (Adj. OR= 4.04, 95% CI=1.15-14.21). This study indicated that child rearing style age 6 months among this population effect to nutritional status for children first year of life. Therefore, health policy and education regarding to appropriate child rearing pattern toward among parents in Thailand should be promoted.

    Effect of efficacy on nursing performance in Indira Gandhi Memorial Hospital, Maldives

    This cross sectional descriptive study was conducted in the government tertiary care hospital in Male’, Maldives. The main objective of this study was to ascertain the relationship between independent variables and nursing performance and to identify the factors which may predict nursing performance. Data was collected using self-administered questionnaire and total of 238 registered nurses participated in this study. Descriptive statistics, Pearson correlation coefficient, and stepwise multiple regression were used for data analysis. The results show that overall task performance as perceived by nurses were at higher level and overall contextual performance were at moderate level. There were correlations between nursing performance and self-efficacy (p<.01), collective efficacy (p<.01). The optimism and competency significantly predicted nurse job performance (p<.01), as did commitment (p<.05), Social support (p<.01) and job autonomy and decision making (p<.05). It is recommended that hospital administration and nursing administration to formulate interventions to improve nurses’ optimism and competency, commitment, social support, job autonomy and decision making.
