    Sharps Injuries among Nurses in a Thai Regional Hospital: Prevalence and Risk Factors

    Background: Sharps injuries (SIs) are one of the most serious occupational accidents among nurses due to the possible severe consequences, such as the transmission of infectious diseases and inducing of mental impairment. Objective: To discover the prevalence of SIs among nurses in a regional hospital in Thailand and to identify factors associated with SIs. Methods: A cross-sectional study was conducted in 2011. Stratified random sampling was used to select the respondents, with wards as the strata and the number of nurses selected proportional to the size of the ward nurse population. 261 self-administered questionnaires were distributed to nurses who used needles, syringes or other sharp medical equipment in their work. Data were analyzed using chi-square tests, correlation analysis and multiple logistic regression analysis. Results: The prevalence of SIs for the previous 12 months was 55.5% among the 250 nurses who returned a completed questionnaire. Of these, 91.1% were with blood. Needles (52.8%) were the main cause of SIs. The reporting rate of SIs to the hospital was 23.8%. SIs had a significant association with each of marital status, work duration, work department, attitude regarding SI prevention, and preventive management. Using multiple logistic regression analysis, attitude was found to be the strongest predictor of SIs when adjusted for other factors. Nurses who had negative attitudes towards prevention of SIs were nearly two times more likely to have SIs than those with positive attitudes (adjusted odds ratio=1.86; 95% CI: 1.03–3.38). Conclusion: The study showed a high prevalence of SIs, but a low reporting rate. This suggests the reporting system requires simplification and also should include a quick response management component. Promoting positive attitudes to SI prevention, and improving the reporting system would reduce SIs.

    Determinants of monthly breast self-examination among women in Samut Songkhram Province, Thailand

    The breast cancer incidence rate is highest among women in Thailand. Monthly breast self-examination (BSE) which is simple, inexpensive and effective to detect breast cancer is recommended for every woman as the first step for early detection. Therefore, cross-sectional study was conducted to identify factors associated with monthly BSE among Thai women aged 20 years old and over in Samut Songkhram Province, Thailand. Data collection procedure was carried out from April to May, 2017. A sample of 405 women was selected by multi-stage cluster sampling to participate to face to face interviews. Chi-square tests and multiple logistic regression were used to examine association between independent variables and monthly BSE. The results of this study showed that only 18.8% of women reported doing monthly BSE while 47.2% never practised BSE. In Chi-square test, variables significantly associated with monthly BSE were family income, health insurance situation, breast cancer knowledge, BSE knowledge, health motivation, BSE training experiences, and mammography experiences. After adjusting for other factors in the multiple logistic regression model, breast cancer knowledge (Adj OR= 3.85; 95% CI= 1.95-7.62), BSE knowledge (Adj OR= 7.31; 95% CI= 2.98-17.93), health motivation (Adj OR= 2.10; 95% CI= 1.14-3.39) and BSE training experiences (Adj OR= 2.85; 95% CI= 1.48-5.49) remained significantly associated with monthly BSE. These findings suggest that providing BSE training to the women is the key in promoting monthly BSE. Therefore, promoting effective BSE training to increase their knowledge about BSE and breast cancer will result in a widespread of monthly BSE among women.

    Predictors of dental service utilization among visually impaired people in Chiang Mai, Thailand

    The purposes of this cross-sectional study were to examine factors associated with utilization of dental health services among visually impaired people and also identify barriers affecting such utilization. A structured questionnaire and an oral examination were used to collect data in January, 2013. In total, 266 visually impaired people participated in the study. Data were analyzed using chi-square tests and multiple logistic regression. The utilization of dental health services in the past year among visually impaired people was 26.3%. Based on the Chi-square tests, education, occupation, perception of oral status, perception of oral health problems and perceptions of the oral health service system were found to be statistically significant associated with dental service utilization. In logistic regression, significant predictors of dental service utilization among visually impaired people included perception of oral status (adj OR = 4.45, 95% CI = 1.41-14.05), perception of oral health problems (adj OR = 4.66, 95% CI = 1.96-11.07) and perception of the dental service system (adj OR = 2.44, 95% CI = 1.32-4.49). The three most commonly reported barriers to dental visits were “no one takes me to the dentist”, “not enough time” and “waiting until the pain gets worse”. The findings suggested that the significant key factor to increase utilization among visually impaired people is providing an appropriate oral health education to improve the knowledge of oral health care, and finally lead to change the perception of oral problems in these people. Furthermore, special dental service system should be established to minimize barriers to care and serve needs of these people.
